Abstract

The invention provides devices for the disruption of one or more layers of skin and methods of their use to administer therapeutic agents, e.g., antigens or drugs. The devices are designed to disrupt a defined area of skin. The defined area can approximate the area that a patch or other suitable vehicle for therapeutic agent, e.g., drug or vaccine, delivery is designed to contact. Exemplary devices employ a mask to define the area to be disrupted. Other devices disrupt a defined area by rotating in place. For devices that employ a mask that is secured to the skin, the invention provides methods of disrupting the stratum corneum by first securing the mask to the skin and then disrupting the skin. For rotating devices, the disrupting member is simply placed against the skin and actuated to effect disruption.

Claims

1. A system comprising a device for disrupting skin, said device comprising a mask and a disrupting member mechanically coupled to said mask, wherein said mask has an opening and comprises a fastener to secure said mask to skin, and said disrupting member is capable of being passed over said opening to disrupt skin exposed therethrough, wherein the movement of said disrupting member is not constrained by said opening.

2. The system of claim 1, further comprising a reservoir containing a therapeutic agent, mechanically coupled to said mask, and capable of delivering said therapeutic agent to said opening.

3. The system of claim 2, wherein said therapeutic agent comprises a drug or an antigen.

4. The system of claim 2, wherein said reservoir is in fluid communication with said disrupting member.

5. The system of claim 2, wherein said reservoir comprises a patch capable of being placed over said opening.

6. The system of claim 5, wherein the decoupling of said disrupting member from said mask exposes said patch.

7. The system of claim 1, wherein said disrupting member comprises an indicator of the amount of force applied to said disrupting member.

8. The system of claim 7, wherein said indicator comprises tactile, auditory, or visual feedback.

9. The system of claim 8, wherein said indicator comprises a snap spring or snap dome.

10. The system of claim 1, wherein said disrupting member is coupled to said mask in a loop.

11. The system of claim 1, wherein said fastener comprises an adhesive.

12. The system of claim 1, wherein said mask further comprises an opening for marking the skin while said mask is adhered to the skin.

13. The system of claim 12, wherein a marking on the skin made through said opening for marking the skin denotes the location of disruption.

14. The system of claim 1, wherein said mask further comprises an indicator disposed to transfer to the skin when the mask is fastened thereto.

15. The system of claim 14, wherein said indicator denotes the location of disruption when transferred to the skin.

16. The system of claim 1, wherein said disrupting member comprises a metal or ceramic abrasive.

17. The system of claim 1, wherein said disrupting member comprises metal blades or protrusions capable of cutting grooves in the skin.

46. The system of claim 1, wherein said disrupting member comprises a disrupting strip slideably coupled to said mask, wherein said disrupting strip is disposed to disrupt skin through said opening as said disrupting strip slides relative to said mask.

47. The system of claim 46, further comprising an indicator of the amount of force applied to said disrupting member.

48. The system of claim 47, wherein said indicator comprises tactile, auditory, or visual feedback.

49. The system of claim 48, wherein said indicator comprises a snap spring or snap dome.

50. The system of claim 46, wherein said fastener comprises an adhesive.

51. The system of claim 46, further comprises an interlock that prevents said disrupting strip from moving unless a minimum amount of force is applied at the start of movement.

52. The system of claim 46, wherein said strip is compressed prior to sliding.

53. The system of claim 46, wherein said disrupting strip comprises a metal or ceramic abrasive.

54. The system of claim 46, wherein said disrupting member comprises metal blades or protrusions capable of cutting grooves in the skin.

55. The system of claim 46, wherein said mask further comprises an opening for marking the skin while said mask is adhered to the skin.

56. The system of claim 55, wherein a marking on the skin made through said opening for marking the skin denotes the location of disruption.

57. The system of claim 46, wherein said mask further comprises an indicator disposed to transfer to the skin when the mask is fastened thereto.

58. The system of claim 57, wherein said indicator denotes the location of disruption when transferred to the skin.

59. The system of claim 46, wherein the lateral edges of said strip are enclosed in said device prior to actuation.

60. The system of claim 59, wherein the lateral edges of said strip are enclosed and are guided during actuation.

61. The system of claim 46, wherein a single actuation of said device produces sufficient skin disruption for treatment.

80. The system of claim 1, further comprising a reservoir comprising a therapeutic agent.

81. The system of claim 80, wherein said reservoir further comprises an adhesive for adhering to skin and a release liner disposed over said adhesive.

82. The system of claim 81, wherein said release liner comprises first, second, and third components, wherein removal of said first component exposes said therapeutic agent and at least a portion of said adhesive.

83. The system of claim 82, wherein said second and third components are disposed for application to the reservoir to skin with two hands.

84. The system of claim 81, wherein said therapeutic agent is in communication with an area disposed within an area of said adhesive.

85. The system of claim 84, wherein an area of no adhesive is disposed between said therapeutic agent and said adhesive.

86. The system of claim 80, wherein said reservoir is shaped for or further comprises an indicator of orientation for placement on a disrupted region of skin to deliver an effective amount of said therapeutic agent.

119. The system of claim 80, wherein said reservoir comprises a patch.

120. The system of claim 119, wherein said therapeutic agent is disposed in a formulation.

121. The system of claim 120, wherein said formulation comprises an antigen, an adjuvant, and a pharmaceutically acceptable carrier.

122. The system of claim 120, wherein said formulation comprises an antigen and a pharmaceutically acceptable carrier.

123. The system of claim 122, wherein said formulation consists essentially of an antigen and one or more pharmaceutically acceptable carriers.

124. The system of claim 123, wherein said formulation is in dry form.

125. A method of inducing an antigen-specific immune response in a subject comprising:

a) treating an area of the skin of the subject with a system of claim 1;

and

b) applying a formulation transcutaneously to the treated area to induce an antigen-specific immune response, wherein the formulation comprises an antigen in an amount effective to induce an antigen-specific immune response and a pharmaceutically acceptable carrier, and wherein treating the area of the skin with the system enhances the immune response.

126. The method of claim 125, wherein the formulation consists essentially of an antigen and one or more pharmaceutically acceptable carriers.

127. The method of claim 126, wherein the formulation is in dry form.

128. The method of claim 125, wherein said formulation further comprises an adjuvant in an amount effective to enhance the immune response to the antigen.
