Abstract
A method of predictive modeling and risk stratification of a medication therapy regimen identifies potentially adverse health outcomes resulting from the medication therapy regimen. Methods of predictive modeling and risk stratification of a medication therapy regimen and intervention thereof are provided herein. The methods of predictive modeling and risk stratification include receiving medical condition and medication information, evaluating the medical condition and medication information to identify an adverse health outcome and evaluating the likelihood of the identified adverse health outcome based on the medical condition and medication information. Persons may be identified from among a plurality of persons for medication therapy regimen intervention based on the likelihood of the adverse health outcome. The methods may also include receiving medical insurance information and creating an intervention severity index based on the medication and medical insurance information, which relates to a degree of urgency in intervening in a medial treatment of a person.
