1. An apparatus comprising:

  • a subcutaneously insertable element that comprises a proximal portion and a distal portion, the distal portion being configured for subcutaneous placement within a human body; and

    a sensor that electrochemically interacts with one or more analytes and generates a signal that is representative of a concentration of the one or more analytes, the sensor being disposed on or within the distal portion of the subcutaneously insertable element;

    a processor that receives and processes the signal from the sensor to determine the concentration of the one or more analytes within the human body; and

    an external device that comprises the processor and that is in mechanical contact with the proximal portion of the subcutaneously insertable element.
