Abstract

A system and method for predicting the effect of patient self-care actions on a disease control parameter. A future disease control parameter value X(t_j) at time t_j is determined from a prior disease control parameter value X(t_i) at time t_i based on an optimal control parameter value R(t_j) at time t_j, the difference between the prior disease control parameter value X(t_i) and an optimal control parameter value R(t_i) at time t_i, and a set of differentials between patient self-care parameters having patient self-care values S_M(t_i) at time t_i and optimal self-care parameters having optimal self-care values O_M(t_i) at time t_i. The differentials are multiplied by corresponding scaling factors K_M. The system includes an input device for entering the patient self-care values S_M(t_i). A memory stores the optimal control parameter values R(t_i) and R(t_j), the prior disease control parameter value X(t_i), the optimal self-care values O_M(t_i), and the scaling factors K_M. A processor in communication with the input device and memory calculates the future disease control parameter value X(t_j). A display is connected to the processor to display the future disease control parameter value X(t_j) to a patient.

Claims

1. A computer system for diabetes management, comprising:

(a) an electronic device for measuring glucose values; and

(b) a personal computer for receiving the glucose values measured by the device, wherein the personal computer comprises;

(i) a microprocessor;

(ii) a memory having digital code embedded therein for programming the microprocessor to calculate diabetes care values and to generate a representation based on one or more optimal glucose values, insulin dose values of a patient, said measured glucose values received from said device, and one or more scaling factors for weighting the impact on a glucose value and that is customizable to an individual patient to illustrate the effect on the glucose value of insulin dose actions performed by the individual patient; and

(iii) a display configured to display information according to said diabetes care values including said representation.

2. The system of claim 1, wherein said digital code is further for programming the microprocessor to calculate said diabetes care values based on carbohydrate intake values.

3. The system of claim 1, wherein said display is configured for displaying multiple values in graphical form.

4. The system of claim 1, wherein said display of said representation is in the form of a visual representation including a figure and enables the patient to consider appropriate self care actions.

5. The system of claim 1, wherein said digital code is further for programming the microprocessor to calculate said diabetes care values based on optimal insulin dose values.

The system of claim 1, wherein said digital code is further for programming the microprocessor to determine said one or more scaling factors from one or more physiological parameters including body mass, metabolism rate, fitness level or hepatic or peripheral insulin sensitivity, or combinations thereof.

7. The system of claim 1, wherein said digital code is further for programming the microprocessor to determine said one or more scaling factors from insulin sensitivity.

8. The system of claim 1, wherein said digital code is further for programming the microprocessor to calculate at least one future glucose value.

9. The system of claim 1, further comprising a server computer connectable to the said personal computer for communicating data between the server computer and the personal computer.

10. The system of claim 1, wherein said digital code is for programming the microprocessor to generate graphs of glucose values versus time.

11. The system of claim 10, wherein the glucose values comprise at least one future glucose value.

12. The system of claim 11, wherein said digital code is further for programming the microprocessor to calculate at least one future glucose value.

13. The system of claim 1, wherein said digital code is further for programming the microprocessor to calculate said diabetes care values based on food intake values.

14. The system of claim 1, wherein said digital code is further for programming the microprocessor to calculate said diabetes care values based on exercise values.

15. The system of claim 1, wherein said display of said diabetes care values or said representation are in the form of a visual representation including tables or graphs, or combinations thereof, and enables the patient to consider appropriate self care actions including injecting units of insulin, food intake actions including intake of grams of carbohydrates, or exercise actions including minutes of exercise or durations of exercise, or combinations thereof.

The system of claim 1, wherein the memory comprises one or more optimal self care values including an optimal food exchange parameter value expressed in grams of carbohydrates, an optimal insulin dose parameter value expressed in units of insulin, or an optimal exercise duration parameter value expressed in minute units of exercise, or combinations thereof.

The system of claim 16, wherein said diabetes care values are respectively determined based on one gram of carbohydrate being expected to raise the patient'"'"'s blood glucose level by 4 mg/dL, one unit of insulin being expected to lower the patient'"'"'s blood glucose level by 40 mg/dL, or fifteen minutes of exercise being expected to lower the patient'"'"'s blood glucose level by 5 mg/dL, or said combinations thereof.

18. The system of claim 1, wherein the system is configured for monitoring and controlling glucose level on a regular basis by the patient outside of a medical clinic.

19. The system of claim 1, wherein the electronic device further comprises a housing, wherein a memory and a microprocessor are housed within said housing, and the electronic device further comprises an output port, wherein said output port is integral with said housing, thereby providing a hand-held, readily transportable device.

20. The system of claim 19, further comprising an electronic data recording device arranged within said housing of said electronic device.

21. The system of claim 19, wherein the output port is configured for communicating measured glucose values electronically to the personal computer.

22. The system of claim 1, further comprising a modem for establishing a communication link to another computer through a communication network.

23. The system of claim 1, wherein electronic device and said personal computer have respective input/output ports for establishing a communication link.

24. The system of claim 1, wherein said digital code is further for programming the microprocessor to display multiple diabetes care values in graphical form.

25. The system of claim 1, wherein said digital code is further for programming said microprocessor to generate a further representation in the form of a figure based on optimal self care actions including values determined by a health care provider.

26. The system of claim 25, wherein said further figure comprises a chart or table or both.

27. The system of claim 1, further comprising an insulin delivery device for delivering insulin in response to information associated with said glucose values or diabetes care values, or both.

28. The system of claim 27, wherein said digital code is further for programming said microprocessor to calculate said diabetes care values based on one or more insulin dose values of said insulin delivery device.

29. The system of claim 28, wherein said insulin dose values comprise units of insulin.

30. The system of claim 27, wherein the electronic device further comprises a housing, wherein a memory and a microprocessor are housed within said housing, and the electronic device further comprises an output port, wherein said output port is integral with said housing, thereby providing a hand-held, readily transportable device.

31. The system of claim 30, further comprising an electronic data recording device arranged within said housing of said electronic device.

32. The system of claim 27, wherein one or more optimal self care values and said one or more optimal glucose values are stored in the memory, entered into the computing device by keypad or entered into the computing device by I/O port, or combinations thereof.

33. The system of claim 27, wherein said insulin delivery device includes a reservoir for storing said insulin for delivery to said patient through a tubular member.

34. The system of claim 33, wherein said reservoir for storing said insulin comprises a barrel and said tubular member includes a cannula for insertion in the skin of said patient.

35. The system of claim 1, wherein said representation comprises a graph.

36. The system of claim 1, wherein said representation comprises a table.

37. The system of claim 1, wherein said representation comprises a chart.

38. The system of claim 1, wherein said representation comprises a plot of glucose values versus time.

39. The system of claim 38, wherein said plot includes one or more predicted future glucose values.

40. The system of claim 1, wherein the electronic device comprises a glucose meter.

41. The system of claim 40, wherein the glucose meter comprises a blood glucose meter for use with glucose test strips.

42. The system of claim 40, wherein the glucose meter comprises a handheld electronic device.

43. The system of claim 1, wherein said glucose values comprises blood glucose values.

44. One or more computer readable media having stored thereon digital code for programming a microprocessor of a personal computer to perform a method of diabetes care management, wherein the method comprises:

(a) receiving and storing one or more optimal glucose values, one or more scaling factors and one or more optimal self care values for a diabetes care patient;

(b) establishing a communication link between said personal computer and an electronic device having stored thereon measured glucose values of said patient;

(c) electronically uploading from the electronic device to the personal computer the measured glucose values;

(d) calculating diabetes care values and generating a representation based on said one or more stored optimal glucose values, said one or more stored scaling factors, one or more self care values of the patient, and said measured glucose values received from said electronic device, in a manner that is customizable to an individual patient to illustrate an effect of self care actions by the patient;

(e) displaying information according to said diabetes care values including said representation; and

(f) thereby monitoring and controlling glucose level on a regular basis by the patient outside of a medical clinic.

45. The one or more media of claim 44, wherein said self care values comprise insulin dose values and said self care actions comprise insulin dose actions.

46. The one or more media of claim 44, wherein the method further comprises calculating said diabetes care values based on carbohydrate intake values.

47. The one or more media of claim 44, wherein the method further comprises displaying multiple values in graphical form.

48. The one or more media of claim 44, wherein the method further comprises calculating said diabetes care values based on optimal insulin dose values.

49. The one or more media of claim 44, wherein the method further comprises calculating said one or more scaling factors from one or more physiological parameters including body mass, metabolism rate, fitness level or hepatic or peripheral insulin sensitivity, or combinations thereof.

50. The one or more media of claim 44, wherein the method further comprises calculating said one or more scaling factors from insulin sensitivity.

51. The one or more media of claim 44, wherein the method further comprises calculating at least one future glucose value.

52. The one or more media of claim 44, wherein the method further comprises connecting the personal computer to a server computer for communicating data between the server computer and the personal computer.

53. The one or more media of claim 44, wherein the method further comprises generating graphs of glucose values versus time.

54. The one or more media of claim 53, wherein the glucose values comprise at least one future glucose value.

55. The one or more media of claim 54, wherein the method further comprises calculating at least one future glucose value.

56. The one or more media of claim 44, wherein the electronic device comprises a glucose meter.

57. The one or more media of claim 56, wherein said establishing a communication link between said personal computer and said glucose meter comprises establishing a communication link through a connection cord.

58. The one or more media of claim 44, wherein the method further comprises receiving into the personal computer by keypad or by I/O port, said one or more optimal self care values and said one or more optimal blood glucose values, or combinations thereof.

59. The one or more media of claim 44, wherein said representation comprises a plot of glucose values versus time.

60. The one or more media of claim 59, wherein said plot includes one or more predicted future glucose values.
