Abstract

The present invention relates to an orthopedic plate and screw system and instruments for surgical fixation of a small bone or bones. The plate facilitates three dimensional contouring to provide for a variety of applications and to accommodate individual variation in bone shape. The plate has a modified x shape including a central trunk portion including one or more screw holes along a longitudinal axis and a set of divergent upper and an oppositely extending set of divergent lower arms, each arm including screw holes which are placed at a radially equal distance but which diverging asymmetrically from the longitudinal axis relative to its paired upper or lower mate. The screws of the system are self-starting, self-tapping screws including the option of partial or full cannulation.

Claims

1. A method of facilitating surgery comprising providing a system of orthopedic plates comprising

a surgical tray presenting an alpha version and a beta version of an orthopedic plate, the plate having an inferior surface which is capable of facing a bone surface in use which forms a segment of a cylinder and an oppositely facing concentric superior surface, the plate having a central trunk portion defining a longitudinal axis extending between a first end and a second end, said plate including at the first end a pair of divergent arms which extend so as to form a left arm and a right arm, each arm including a screw hole joined to the trunk portion by a link, each arm of the pair having a longitudinal axis defining an angle with respect to the longitudinal axis of the trunk area and the angle of the left arm being α

and the angle of the right arm being β

where α

is different than β and

the beta version of the plate is a mirror image of the alpha version of the plate; and

wherein the alpha version of the plate is provided in at least two alpha versions, a first version having one screw hole in the central trunk, and a second version having other than one screw hole in the central trunk, and wherein the beta version of the plate is provided in at least two beta versions, a first version having one screw hole in the central trunk, and a second version having other than one screw hole in the central trunk.

2. A method of facilitating surgery as set forth in claim 1, wherein each of the alpha and the beta plates include two sets of arms.

3. A method of facilitating surgery as set forth in claim 2, wherein the alpha version of the plate is provided in at least three alpha versions, a first alpha version having one screw hole in the central trunk, a second alpha version having no screw holes in the central trunk, and the third alpha version having from two to four screw holes in the central trunk and wherein the beta version of the plate is provided in at least three beta versions, a first beta version having one screw hole in the central trunk, a second beta version having no screw holes in the central trunk, and the third beta version having from two to four screw holes in the central trunk.

4. A method of facilitating surgery as set forth in claim 3, wherein at least one version of the alpha plate and at least one version of the beta plate has a radius of curvature on the inferior surface.

A method of facilitating surgery as set forth in claim 4, further including screws having a threaded shaft and a head wherein the screw holes and the screw heads have a mating interface such that the screws can engage the screw hole so as to allow a plurality of angular orientations of the screw axis.

6. A method of facilitating surgery as set forth in claim 5, wherein the mating interface between the screw heads and the screw holes allow for at least 20°

of conical orientation of the screw axis in the screw hole.

7. A method of facilitating surgery as set forth in claim 6, wherein the mating interface between the screw heads and the screw holes allow for at least 25°

of conical orientation of the screw axis in the screw hole.

8. A method of facilitating surgery as set forth in claim 7, wherein the mating interface between the screw heads and the screw holes allow for at least 30°

of conical orientation of the screw axis in the screw hole.

9. A method of facilitating surgery as set forth in claim 8, wherein the central trunk of the alpha plate and the central trunk of the beta plate each have a compression slot.

10. A method of enabling a surgery on one or more of a set of bones comprising the ulna, radius, tibia, fibula, metacarpals, carpals, metatarsals, tarsals, and phalanges comprising

providing a surgical tray including a pre-contoured plate which is a Y-shaped plate, the plate comprising a trunk defining a longitudinal axis with a first end and an opposing second end, and a first arm and a second arm extending from the first end of the trunk, the trunk having a constant radial curve in the z direction about the longitudinal axis, and the first arm and the second arm following the radial curve of the trunk so as to spiral about the longitudinal axis toward each other,

the first arm having an ear with at least one screw hole defining a first screw axis perpendicular to a tangent to the top surface of the first ear, the first ear being attached to the trunk by a linking section having a waist, a first angle and a first length being defined by a line from the center of the first arm screw hole to the intersection of the longitudinal axis of the trunk,

the second arm having a second ear with at least one second screw hole defining a second screw axis perpendicular to a tangent to the top surface of the second ear, the second ear being attached to the trunk by a linking section having a waist, a second angle and a second length being defined by a line from the center of the second arm screw hole to the intersection of the longitudinal axis of the trunk.

the first angle and the first length being different from the second angle and the second length whereby the first screw axis and the second screw axis converge toward the inferior side of the plate but do not intersect, and

the tray further including a plate bender which will bend the linking section of the first arm and the second arm without deforming the screw hole of the arm.

11. A method of enabling a surgery as set forth in claim 10, wherein the curve of the trunk and of the first arm and of the second arm is constant along the longitudinal axis.

12. A method of enabling a surgery as set forth in claim 11, wherein the curve is a portion of a circle and the plate defines a segment of a cylinder.

13. A method of enabling a surgery as set forth in claim 10, wherein the waist of the linking section of the first arm and of the second arm is configured to bend relative to the trunk section in response to a force applied before or during surgery without deforming the screw hole.

14. A method of enabling a surgery as set forth in claim 10, wherein the trunk has a through hole.

15. A method of enabling a surgery as set forth in claim 14, wherein the trunk has at least two through holes and a waist area between the through holes which encourages bending of the waist area in response to a force applied before or during surgery.

16. A method of enabling a surgery as set forth in claim 10, wherein the trunk has a compression slot.

17. A method of enabling a surgery as set forth in claim 16, wherein the compression slot comprises a slot having an internal edge which includes a shoulder that slopes toward the inferior side of the plate as it extends away from the first end of the trunk.

18. A method of enabling a surgery as set forth in claim 17, wherein the superior surface of the trunk includes a visual indication of the direction in which the compression slot can be used to apply compression.

19. A method of enabling a surgery as set forth in claim 16, further including screws having a threaded shafts and a head wherein the screw holes and the screw heads have a mating interface such that the screws can engage the screw hole so as to allow a plurality of angular orientations of the screw axis.

20. A method of enabling a surgery on one or more of a set of bones comprising the ulna, radius, tibia, fibula, metacarpals, carpals, metatarsals, tarsals, and phalanges comprising

providing a surgical tray including each of a set of alpha and a set of beta pre-contoured plates, each plate being a Y-shaped plate, the plate comprising a trunk defining a longitudinal axis with a first end and an opposing second end, and a first arm and a second arm extending from the first end of the trunk, the trunk having a constant radial curve in the z direction about the longitudinal axis, and the first arm and the second arm following the radial curve of the trunk so as to spiral about the longitudinal axis toward each other so that the plate defines a segment of a cylinder,

the first arm having an ear with at least one screw hole defining a first screw axis perpendicular to a tangent to the top surface of the first ear, the first ear being attached to the trunk by a linking section having a waist, a first angle and a first length being defined by a line from the center of the first arm screw hole to the intersection of the longitudinal axis of the trunk,

the second arm having a second ear with at least one second screw hole defining a second screw axis perpendicular to a tangent to the top surface of the second ear, the second ear being attached to the trunk by a linking section having a waist, a second angle and a second length being defined by a line from the center of the second arm screw hole to the intersection of the longitudinal axis of the trunk, the first angle and the first length being different from the second angle and the second length whereby the first screw axis and the second screw axis converge toward the inferior side of the plate but so not intersect,

the alpha plate being a mirror image of the beta plate, and each of the alpha and the beta sets including at least a first plate having a trunk with a single through hole and a second plate having a trunk having at least two through holes;

and

the tray further comprising a plate bender which will bend the linking section of the first arm and the second arm without deforming the screw hole of the arm.

21. A method of enabling a surgery as set forth in claim 20, wherein each of the alpha set and the beta set further includes an X-shaped plate having mirror symmetry about a plane transverse to its longitudinal axis, the plate comprising a trunk defining a longitudinal axis with a first end and an opposing second end, and a first set of a first arm and a second arm extending from the first end of the trunk and a second set of a first arm and a second arm extending from the second end of the trunk, the trunk having a constant radial curve in the z direction about the longitudinal axis and each of the first arm and the second arm of the first set and the second set following the radial curve of the trunk so as to spiral about the longitudinal axis toward each other so that the plate defines a segment of a cylinder,

the first arm of each arm set having an ear with at least one screw hole defining a first screw axis perpendicular to a tangent to the top surface of the first ear,

the first ear being attached to the trunk by a linking section having a waist, a first angle and a first length being defined by a line from the center of the first arm screw hole to the intersection of the longitudinal axis of the trunk,

the second arm of each arm set having a second ear with at least one second screw hole defining a second screw axis perpendicular to a tangent to the top surface of the second ear, the second ear being attached to the trunk by a linking section having a waist, a second angle and a second length being defined by a line from the center of the second arm screw hole to the intersection of the longitudinal axis of the trunk,

the first angle and the first length being different from the second angle and the second length for each arm set whereby the first screw axis and the second screw axis of each arm set converge toward the inferior side of the plate but do not intersect.

22. A method of performing a surgery as set forth in claim 21, wherein the tray further includes screws having a threaded shafts and a head wherein the screw holes and the screw heads have a mating interface such that the screws can engage the screw hole so as to allow a plurality of angular orientations of the screw axis.

23. A method of performing a surgery on one or more of a set of bones comprising the ulna, radius, tibia, fibula, as well as the metacarpals, carpals, metatarsals, tarsals, and phalanges comprising

accessing the bone by opening a wound;

selecting a pre-contoured plate which is a Y-shaped plate, the plate comprising a trunk defining a longitudinal axis with a first end and an opposing second end, and a first arm and a second arm extending from the first end of the trunk the trunk having a constant radial curve in the z direction about the longitudinal axis and the first arm and the second arm following the radial curve of the trunk so as to spiral about the longitudinal axis toward each other, the first arm having an ear with at least one screw hole defining a first screw axis perpendicular to a tangent to the top surface of the first ear, the first ear being attached to the trunk by a linking section having a waist, a first angle and a first length being defined by a line from the center of the first arm screw hole to the intersection of the longitudinal axis of the trunk, the second arm having a second ear with at least one second screw hole defining a second screw axis perpendicular to a tangent to the top surface of the second ear, the second ear being attached to the trunk by a linking section having a waist, a second angle and a second length being defined by a line from the center of the second arm screw hole to the intersection of the longitudinal axis of the trunk, the first angle and the first length being different from the second angle and the second length whereby the first screw axis and the second screw axis converge toward the inferior side of the plate but so not intersect;

using a plate bender to bend the linking section of the first arm and the second arm without deforming the screw hole of the arm,

attaching the bent plate to the bone, and

surgically closing the wound.

24. A method of performing a surgery as set forth in claim 23, wherein the trunk has a through hole.

25. A method of performing a surgery as set forth in claim 24, wherein the trunk has at least two through holes and a waist area between the through holes which encourages bending of the waist area in response to a force applied before or during surgery.

26. A method of performing a surgery as set forth in claim 25, wherein the trunk has a compression slot.

27. A method of performing a surgery as set forth in claim 26, wherein the compression slot comprises a slot having an internal edge which includes a shoulder that slopes toward the inferior side of the plate as it extends away from the first end of the trunk.

28. A method of performing a surgery as set forth in claim 27, wherein the superior surface of the trunk includes a visual indication of the direction in which the compression slot can be used to apply a compression.

29. A method of performing a surgery as set forth in claim 23, wherein the tray further includes screws having a threaded shafts and a head wherein the screw holes and the screw heads have a mating interface such that the screws can engage the screw hole so as to allow a plurality of angular orientations of the screw axis.

30. A method of performing a surgery as set forth in claim 29, further including at least one screw having a threaded shaft and a head and wherein each of the screw holes in the first arm and the second arm is inwardly rounded and the head of the screw is outwardly rounded to enable the screw to be implanted through the screw hole at a variety of angles

31. A method of performing a surgery as set forth in claim 30, wherein the mating interface between the screw heads and the screw holes allow for at least 20°

of conical orientation of the screw axis in the screw hole.

32. A method of performing a surgery as set forth in claim 23, wherein the alpha plate is provided in at least two versions, a first version having one screw hole in the central trunk, and a second version having other than one screw hole in the central trunk, and central trunk.

33. A method of performing a surgery as set forth in claim 32, wherein the alpha plate is provided in at least three versions, a first version having one screw hole in the central trunk, a second version having no screw holes in the central trunk, and the third version having from two to four screw holes in the central trunk and wherein the beta plate is provided in at least three versions, a first version having one screw hole in the central trunk, a second version having no screw holes in the central trunk, and the third version having from two to four screw holes.
