Abstract

A computer implemented system and method facilitates a cycle of generation, sharing, and refinement of volumes related to stimulation of anatomical tissue, such as brain or spinal cord stimulation. Such volumes can include target stimulation volumes, side effect volumes, and volumes of estimated activation. A computer system and method also facilitates analysis of groups of volumes, including analysis of differences and/or commonalities between different groups of volumes.

Claims

1. A computer-implemented method, comprising:

obtaining atlas registration information regarding a patient from a surgical planning module in which a surgical trajectory for implantation of a leadwire is generated;

obtaining from the surgical planning module a target volume to be activated;

determining, by a processor, stimulation parameter settings that are estimated to provide stimulation in at least a portion of the target volume;

analyzing, by the processor, effects of stimulations performed on a plurality of patients of a patient population;

based on the analysis, determining, by the processor, a new target volume; and

providing the new target volume for access by the surgical planning module.

2. The computer-implemented method of claim 1, wherein obtaining atlas registration information comprises identifying an AC-PC (anterior commissure—

posterior commissure) in an image of the patient.

3. The computer-implemented method of claim 1, wherein obtaining atlas registration information comprises obtaining an image of the patient and registering the image to an atlas.

The computer-implemented method of claim 1, wherein obtaining atlas registration information comprises comparing the patient to a plurality of population groups of the patient population, wherein an atlas is associated with each of the population groups, and selecting the atlas of the population group that is most similar to the patient.

5. The computer-implemented method of claim 4, wherein obtaining atlas registration information comprises transforming an image of the patient to register the image to the atlas of the population group that is most similar to the patient.

6. The computer-implemented method of claim 1, wherein determining a new target volume comprises obtaining user input of at least one clinical effect;

obtaining a plurality of estimated volumes of activation (VOA) associated with stimulations of the patient population, which VOAs are associated with the at least one clinical effect; and

based on the VOAs, determining the new target volume.

7. The computer-implemented method of claim 6, wherein determining the new target volume comprises selecting points for the new target volume that belong to a threshold number or percentage of the VOAs.

8. The computer-implemented method of claim 6, further comprising obtaining a rating for each of the VOAs with respect to the at least one clinical effect, wherein determining the new target volume comprises selecting points for the new target volume that have a threshold score based on the VOAs containing the point weighted by the rating of the VOA.

9. The computer-implemented method of claim 6, further comprising filtering the VOAs by a time of day when stimulation of the VOA was performed.

10. The computer-implemented method of claim 6, further comprising filtering the VOAs by a medication used by a patient associated with the VOA.

11. The computer-implemented method of claim 6, wherein the at least one clinical effect comprises a therapeutic effect.

12. The computer-implemented method of claim 6, wherein the at least one clinical effect comprises a side effect.

13. A system, comprising:

at least one computer processor, the at least one computer processor configured and arranged to perform the following acts;

obtaining a target volume to be activated based on a surgical trajectory for implantation of a leadwire;

determining stimulation parameter settings that are estimated to provide stimulation in at least a portion of the target volume;

analyzing effects of stimulations performed on a plurality of patients of a patient population;

based on the analysis, determining a new target volume; and

providing the new target volume.

14. The system of claim 13, wherein the at least one computer processor is configured and arranged to perform the following additional act:

obtaining atlas registration information regarding the surgical trajectory for implantation of the leadwire.

15. The system of claim 14, wherein obtaining atlas registration information comprises obtaining an image of the patient and registering the image to an atlas.

16. The system of claim 14, wherein obtaining atlas registration information comprises comparing the patient to a plurality of population groups of the patient population, wherein an atlas is associated with each of the population groups, and selecting the atlas of the population group that is most similar to the patient.

17. The system of claim 16, wherein obtaining atlas registration information comprises transforming an image of the patient to register the image to the atlas of the population group that is most similar to the patient.

18. The system of claim 13, wherein determining a new target volume comprises obtaining user input of at least one clinical effect;

obtaining a plurality of estimated volumes of activation (VOA) associated with stimulations of the patient population, which VOAs are associated with the at least one clinical effect; and

based on the VOAs, determining the new target volume.

19. The system of claim 18, wherein determining the new target volume comprises selecting points for the new target volume that belong to a threshold number or percentage of the VOAs.

20. The system of claim 18, further comprising obtaining a rating for each of the VOAs with respect to the at least one clinical effect, wherein determining the new target volume comprises selecting points for the new target volume that have a threshold score based on the VOAs containing the point weighted by the rating of the VOA.
