Abstract

The present invention relates to apparatuses and methods of using a resection line guide in various medical procedures. A resection line guide may include a first clamp member and a second clamp member configured to be positioned on a first side and a second side generally opposite that of the first side of an anatomical structure, such as, for example, a stomach. The clamp members may be configured to provide a clamping force on the stomach to secure the guide to the stomach. Further, at least one flexible member may be operatively coupled to the clamp members. The flexible member may be configured to be tensioned so as to provide at least a portion of the clamping force on the stomach.

43. A guide for guiding a medical instrument during a medical procedure on an anatomical structure, the guide comprising:

a first clamp member configured to be positioned on a first side of the anatomical structure;

a second clamp member configured to be positioned on a second side of the anatomical structure generally opposite that of the first side; and

a first biasing mechanism extending between the first and second clamp members and a second biasing mechanism extending between the first and second clamp members; and

wherein the first and second clamp members are configured to provide a clamping force on the anatomical structure to secure the guide to the anatomical structure;

wherein at least one of the clamp members is configured to cooperate with the medical instrument in order to guide and support the medical instrument during the medical procedure;

wherein the first and second clamp members are biased towards each other to provide at least a portion of the clamping force on the anatomical structure; and

wherein the first biasing mechanism extends between the first and second clamp members adjacent a first end of the first and second clamp members and the second biasing mechanism extends between the first and second clamp members adjacent a second end of the first and second clamp members.

361. A medical device for performing a medical procedure comprising:

a manipulator including a shaft;

a resection line guide being coupled to the shaft and being configured to clamp an anatomical structure in the human body; and

a flexible member operably coupled to the manipulator and extending through the shaft to the resection line guide, wherein the manipulator is configured to place the flexible member in tension so that the resection line guide imposes a clamping force on the anatomical structure.

362. The medical device of claim 361, wherein the resection line guide includes:

a first clamp member for contacting the anatomical structure; and

a second clamp member for contacting the anatomical structure generally opposite the anatomical structure to that of the first clamp member, one of the first clamp member and the second clamp member being coupled to the shaft,

wherein the flexible member extends from the first clamp member to the second clamp member so that the flexible member moves the first clamp member toward the second clamp member.

364. The medical device of claim 361, wherein the resection line guide is coupled to the shaft at a ball-and-socket type joint.

365. The medical device of claim 361, wherein the resection line guide is coupled to the shaft at a joint.

366. The medical device of claim 365, wherein the joint is configured to be locked to fix an orientation between the shaft and the resection line guide.

367. The medical device of claim 365, wherein the joint is in a normally locked configuration.

369. The medical device of claim 367, wherein the joint is operably coupled to the manipulator, and wherein the manipulator includes a mechanism for selectively unlocking the joint.

385. The medical device of claim 365, wherein the manipulator includes a mechanism for locking the joint including a knob that is rotatable relative to the shaft to lock and unlock the joint.

394. The medical device of claim 361, wherein the manipulator includes a housing and a clamping mechanism operable to selectively tension the flexible member.

399. The medical device of claim 394, wherein the clamping mechanism includes a knob rotatable so as to withdrawal the flexible member from the resection line guide.

400. The medical device of claim 399, wherein the clamping mechanism includes a cam rod, the flexible member being secured to the cam rod, and the knob being operably coupled to the cam rod such that rotation of the knob relative to the housing moves the cam rod relative to the housing.

407. The medical device of claim 361, wherein the manipulator includes a housing, a clamping mechanism operable to selectively apply tension to the flexible member in an engaged position, and a stop and release mechanism for maintaining the clamping mechanism in the engaged position.

416. The medical device of claim 361, wherein the resection line guide is configured to provide a first stage clamping force on the anatomical structure, the first stage clamping force configured to couple the resection line guide to the anatomical structure while permitting the clamp members to be moved relative to the anatomical structure.

417. The medical device of claim 416, wherein the first stage clamping force is between about 0.5 g/mm² and about 4 g/mm².

418. The medical device of claim 416, wherein the manipulator includes a spring reel for letting out a length of the flexible member and taking up a length of the flexible member that is configured to provide the first stage clamping force.
