- two or more non-conductive substrates;
one or more electrical leads;
two or more conductive patient contacts wherein at least one patient contact is formed on each non-conductive substrate;
wherein each electrical lead is configured to be electrically connected to at least one of the two or more patient contacts; and
wherein the two or more patient contacts are arranged in a configuration with at least one patient contact on each non-conductive substrate and each patient contact is electrically isolated from the other patient contacts by the non-conductive substrate.
