Abstract

A method of managing insulin includes receiving blood glucose measurements on a computing device from a glucometer. The blood glucose measurements are separated by a time interval. The method includes determining, by the computing device, an insulin dose rate based on the blood glucose measurements and determining a blood glucose drop rate based on the blood glucose measurements and the time interval. The method also includes determining a blood glucose percentage drop based on the blood glucose measurements. The method includes decreasing the time interval between blood glucose measurements by the glucometer when the blood glucose drop rate is greater than a threshold drop rate, and decreasing the time interval between blood glucose measurements by the glucometer when the blood glucose percentage drop is greater than a threshold percentage drop.

Claims

1. A method comprising:

receiving, at data processing hardware, sequential blood glucose measurements of a patient, the sequential blood glucose measurements comprising a current blood glucose measurement measured at a current time;

executing, by the data processing hardware, an intravenous treatment process configured to display on a screen in communication with the data processing hardware a graphical user interface, wherein the intravenous treatment process is configured to;

display, in the graphical user interface, a countdown timer indicating a remaining time until a next time of a next scheduled blood glucose measurement for the patient;

display, in the graphical user interface, a patient information input window;

receive, in the patient information input window, a target blood glucose range for the patient comprising a range of blood glucose values between and including a lower limit blood glucose value and an upper limit blood glucose value greater than the lower limit blood glucose value; and

receive, in the patient information input window, a specified number of consecutive blood glucose measurements of the patient within the target blood glucose range to determine a blood glucose concentration for the patient is stable in the target blood glucose range;

determining, by the data processing hardware, whether the current blood glucose measurement and one or more previous blood glucose measurements equal the specified number of consecutive blood glucose measurements within the target blood glucose range; and

determining, by the data processing hardware, a next time interval from the current time until the next time of the next scheduled blood glucose measurement for the patient based on the current blood glucose measurement, the next time interval comprising a long time interval when the current blood glucose measurement and the one or more previous blood glucose measurements equal the specified number of consecutive blood glucose measurements within the target blood glucose range.

2. The method of claim 1, wherein the intravenous treatment process is further configured to display, in the graphic user interface, a blood glucose due message at the end of the next time interval indicating that the next scheduled blood glucose measurement for the patient is due.

3. The method of claim 1, wherein the intravenous treatment process is further configured to receive, in the graphical user interface, a blood glucose input from the medical professional for a next blood glucose measurement measured at a subsequent time after the current time.

The method of claim 1, wherein the intravenous treatment process is further configured to receive, in the graphical user interface, a recommended duration of time required for the current blood glucose measurement within the target blood glucose range to determine the blood glucose concentration for the patient is stable in the target blood glucose range.

5. The method of claim 4, further comprising:

determining, by the data processing hardware, whether the current blood glucose measurement has been within the blood glucose target range for a duration of time greater than the recommended duration of time; and

when the current blood glucose measurement has been within the blood glucose target range for the duration of time greater than the recommended duration of time, setting, by the data processing hardware, the next time interval to the long time interval.

6. The method of claim 1, wherein the intravenous treatment process is further configured to receive, in the graphical user interface:

a threshold hypoglycemia blood glucose value for the patient, the threshold hypoglycemia blood glucose value less than the lower limit blood glucose value of the target blood glucose range;

a low blood glucose percentage drop limit for the patient when the current blood glucose measurement is less than the lower limit blood glucose value of the target blood glucose range;

a regular blood glucose percentage drop limit for the patient when the current blood glucose measurement is greater than or equal to the lower limit blood glucose value of the target blood glucose range; and

a blood glucose drop rate limit for the patient.

7. The method of claim 6, wherein determining the next time interval comprises:

determining a blood glucose percentage drop based on the current blood glucose measurement and a previous blood glucose measurement of the sequential blood glucose measurements, the previous blood glucose measurement measured at a previous time earlier than the current time; and

setting the next time interval to a short time interval greater than the hypoglycemia time interval when;

the current blood glucose measurement is greater than the threshold hypoglycemia blood glucose value and less than the lower limit blood glucose value of the target blood glucose range and the blood glucose percentage drop is greater than the low blood glucose percentage drop limit;

or

the current blood glucose measurement is greater than or equal to the lower limit blood glucose value of the target blood glucose range and the blood glucose percentage drop is greater than the regular blood glucose percentage drop limit.

8. The method of claim 6, further comprising when the current blood glucose measurement is less than the threshold hypoglycemia blood glucose value, determining, by the data processing hardware, a recommended dose of intravenous glucose for administration to the patient intravenously or oral glucose for ingestion by the patient.

9. The method of claim 1, further comprising:

determining, by the data processing hardware, an intravenous insulin infusion rate based on the current blood glucose measurement,

wherein the intravenous treatment process is further configured to display, in the graphical user interface, the intravenous infusion rate indicating a number of units of insulin within a configured time interval for administration to the patient intravenously using an insulin administration device.

10. The method of claim 9, further comprising sending the intravenous insulin infusion rate from the data processing hardware to the insulation administration device in communication with the data processing hardware, and when the insulin administration device receives the intravenous insulin infusion rate, the insulin administration device configured to administer insulin to the patient intravenously using the intravenous insulin infusion rate.

11. The method of claim 9, wherein determining the intravenous insulin infusion rate comprises calculating:

wherein IIR is the intravenous insulin infusion rate, BG is the current blood glucose measurement, K is a constant, and M is a multiplier.

12. The method of claim 11, further comprising, in response to receiving an indication of patient solid food consumption, increasing the intravenous insulin infusion rate and maintaining the multiplier unchanged for at least two subsequent next time intervals.

13. A system comprising:

a dosing controller including data processing hardware and memory hardware in communication with the data processing hardware, the dosing controller configured to perform operations comprising;

receiving sequential blood glucose measurements of a patient, the sequential blood glucose measurements comprising a current blood glucose measurement measured at a current time;

executing an intravenous treatment process configured to display on a screen in communication with the data processing hardware a graphical user interface, wherein the intravenous treatment process is configured to;

display, in the graphical user interface, a countdown timer indicating a remaining time until a next time of a next scheduled blood glucose measurement for the patient;

display, in the graphical user interface, a patient information input window;

receive, in the patient information input window, a target blood glucose range for the patient comprising a range of blood glucose values between and including a lower limit blood glucose value and an upper limit blood glucose value greater than the lower limit blood glucose value; and

receive, in the patient information input window, a specified number of consecutive blood glucose measurements of the patient within the target blood glucose range to determine a blood glucose concentration for the patient is stable in the target blood glucose range;

determining whether the current blood glucose measurement and one or more previous blood glucose measurements equal the specified number of consecutive blood glucose measurements within the target blood glucose range; and

determining a next time interval from the current time until the next time of the next scheduled blood glucose measurement for the patient based on the current blood glucose measurement, the next time interval comprising a long time interval when the current blood glucose measurement and the one or more previous blood glucose measurements equal the specified number of consecutive blood glucose measurements within the target blood glucose range.

14. The system of claim 13, wherein the intravenous treatment process is further configured to display, in the graphic user interface, a blood glucose due message at the end of the next time interval indicating that the next scheduled blood glucose measurement for the patient is due.

15. The system of claim 13, wherein the intravenous treatment process is further configured to receive, in the graphical user interface, a blood glucose input from the medical professional for a next blood glucose measurement measured at a subsequent time after the current time.

16. The system of claim 13, wherein the intravenous treatment process is further configured to receive, in the graphical user interface, a recommended duration of time required for the current blood glucose measurement within the target blood glucose range to determine the blood glucose concentration for the patient is stable in the target blood glucose range.

17. The system of claim 16, wherein the operations further comprise:

determining whether the current blood glucose measurement has been within the blood glucose target range for a duration of time greater than the recommended duration of time; and

when the current blood glucose measurement has been within the blood glucose target range for the duration of time greater than the recommended duration of time, setting the next time interval to the long time interval.

18. The system of claim 13, wherein the intravenous treatment process is further configured to receive, in the graphical user interface:

a threshold hypoglycemia blood glucose value for the patient, the threshold hypoglycemia blood glucose value less than the lower limit blood glucose value of the target blood glucose range;

a low blood glucose percentage drop limit for the patient when the current blood glucose measurement is less than the lower limit blood glucose value of the target blood glucose range;

a regular blood glucose percentage drop limit for the patient when the current blood glucose measurement is greater than or equal to the lower limit blood glucose value of the target blood glucose range; and

a blood glucose drop rate limit for the patient.

19. The system of claim 18, wherein determining the next time interval comprises:

determining a blood glucose percentage drop based on the current blood glucose measurement and a previous blood glucose measurement of the sequential blood glucose measurements, the previous blood glucose measurement measured at a previous time earlier than the current time; and

setting the next time interval to a short time interval greater than the hypoglycemia time interval when;

the current blood glucose measurement is greater than the threshold hypoglycemia blood glucose value and less than the lower limit blood glucose value of the target blood glucose range and the blood glucose percentage drop is greater than the low blood glucose percentage drop limit;

or

the current blood glucose measurement is greater than or equal to the lower limit blood glucose value of the target blood glucose range and the blood glucose percentage drop is greater than the regular blood glucose percentage drop limit.

20. The system of claim 18, wherein the operations further comprise, when the current blood glucose measurement is less than the threshold hypoglycemia blood glucose value, determining a recommended dose of intravenous glucose for administration to the patient intravenously or oral glucose for ingestion by the patient.

21. The system of claim 13, wherein the operations further comprise:

determining an intravenous insulin infusion rate based on the current blood glucose measurement,

wherein the intravenous treatment process is further configured to display, in the graphical user interface, the intravenous infusion rate indicating a number of units of insulin within a configured time interval for administration to the patient intravenously using an insulin administration device.

22. The system of claim 21, wherein the operations further comprise sending the intravenous insulin infusion rate to the insulation administration device in communication with the dosing controller, and when the insulin administration device receives the intravenous insulin infusion rate, the insulin administration device configured to administer insulin to the patient intravenously using the intravenous insulin infusion rate.

23. The system of claim 21, wherein determining the intravenous insulin infusion rate comprises calculating:

24. The system of claim 23, wherein the operations further comprise, in response to receiving an indication of patient solid food consumption, increasing the intravenous insulin infusion rate and maintaining the multiplier unchanged for at least two subsequent next time intervals.
