Abstract

The device senses Natural Heart Acceleration (NHA), isolating at least one segment (t) thereof corresponding to an isovolumetric phase, such as isovolumetric contraction (P1, P2) and/or isovolumetric relaxation (P3, P4). Preferentially, the action of isolation of the aforesaid segment is made starting from the electrocardiogram (ECG) signal. Among other things, the device may be used to carry out pacing functions (pacemaker, defibrillator, pacer cardioverter defibrillator) and/or to pick up, possibly by telemetry, data on the myocardial function and/or also to control a system of drug administration.

Claims

1. A method of determining myocardial function of a patient'"'"'s heart, comprising:

(a) sensing an ECG of the patient;

(b) generating a signal representative of the ECG;

(c) sensing a natural heart acceleration of the patient;

(d) generating a signal representative of the natural heart acceleration; and

(e) selecting at least one time segment of the natural heart acceleration signal based on a predetermined portion of the ECG signal.

2. The method of claim 1 wherein the predetermined portion of the ECG signal is an isovolumetric phase of the patient'"'"'s heart.

3. The method of claim 2 wherein the isovolumetric phase is the isovolumetric contraction phase.

4. The method of claim 2 wherein the isovolumetric phase is the isovolumetric relaxation phase.

5. The method of claim 1 wherein the predetermined portion of the ECG signal is the QRS.

6. The method of claim 1 wherein the predetermined portion of the ECG signal is the T wave.

7. The method of claim 1 further comprising:

(f) measuring a heart beat rate; and

(g) varying the length of the time segment of the natural heart acceleration signal based on the heart beat rate.

8. The method of claim 7 wherein the length of the time segment of the natural heart acceleration signal is decreased when the heart beat rate increases.

9. The method of claim 7 wherein the length of the time segment of the natural heart acceleration signal is increased when the heart beat rate decreases.

10. The method of claim 1 further comprising:

(h) detecting arrhythmic events from the ECG signal; and

(i) discarding the time segment of the natural heart acceleration signal corresponding to the arrhythmic events.

11. The method of claim 1 further comprising:

(j) processing at least one time segment of the natural heart acceleration signal.

12. The method of claim 11 further comprising:

(k) delivering therapy to the heart based on the processed time segment of the natural heart acceleration signal.

13. An apparatus for determining the myocardial function of a patient'"'"'s heart, comprising:

(a) a natural heart acceleration sensor adapted to be coupled with the patient'"'"'s heart;

(b) an ECG sensor adapted to be coupled with the patient'"'"'s heart; and

(c) a processor operably connected to the natural heart acceleration sensor and the ECG sensor;

wherein, the natural heart acceleration sensor measures the natural heart acceleration of the patient'"'"'s heart, the ECG sensor measures the ECG of the patient'"'"'s heart, the processor generates a first signal based on the measured natural heart acceleration, generates a second signal based on the measured ECG, and selects a time segment of the natural heart acceleration signal based on a predetermined portion of the ECG signal.

14. The apparatus of claim 13 wherein the predetermined portion of the ECG signal is an isovolumetric phase of the patient'"'"'s heart.

15. The method of claim 14 wherein the isovolumetric phase is the isovolumetric contraction phase.

16. The method of claim 14 wherein the isovolumetric phase is the isovolumetric relaxation phase.

17. The apparatus of claim 14 further comprising a cardiac pacer adapted to be operably connected to the patient'"'"'s heart, wherein the processor selects a time segment of the natural heart acceleration signal following a pulse from the cardiac pacer.

18. The apparatus of claim 14 wherein the processor measures the heart beat rate of the patient'"'"'s heart based on the ECG signal and varies the length of the time segment of the NHA signal based on the heart beat rate.

19. The apparatus of claim 14 wherein the processor identifies individual arrhythmic events based on the ECG signal and discards the corresponding time segment of the natural heart acceleration signal.

20. The apparatus of claim 14 further including a therapeutic device operably connected to the processor.

21. The apparatus of claim 20 wherein the therapeutic device is selected from the group consisting of a pacemaker, a defibrillator, a PCD, or a drug dispenser.

22. The apparatus of claim 13 wherein the predetermined portion of the ECG signal is the QRS.

23. The method of claim 13 wherein the predetermined portion of the ECG signal is the T wave.

24. The apparatus of claim 13 further including a transmitter operably connected to the processor.

25. A method of providing therapy to a patient'"'"'s heart comprising:

(a) sensing a natural heart acceleration of the patient;

(b) generating a signal representative of the natural heart acceleration;

(c) selecting at least one time segment of the natural heart acceleration signal; and

(d) delivering therapy to the heart of the patient based on the selected time segment of the natural heart acceleration signal.

26. The method of claim 25 wherein the at least one time segment is selected in an isovolumetric phase of the patient'"'"'s heart.

27. The method of claim 26 wherein the isovolumetric phase is the isovolumetric contraction phase.

28. The method of claim 26 wherein the isovolumetric phase is the isovolumetric relaxation phase.

29. The method of claim 25 further comprising:

(e) sensing an ECG of the patient;

(f) generating a signal representative of the ECG; and

wherein the selection of the at least one time segment of the natural heart acceleration signal is based on a predetermined portion of the ECG signal.

30. The method of claim 29 wherein the predetermined portion of the ECG signal is an isovolumetric phase of the patient'"'"'s heart.

31. The method of claim 30 wherein the isovolumetric phase is the isovolumetric contraction phase.

32. The method of claim 30 wherein the isovolumetric phase is the isovolumetric relaxation phase.

33. The method of claim 29 wherein the predetermined portion of the ECG signal is the QRS.

34. The method of claim 29 wherein the predetermined portion of the ECG signal is the T wave.

35. The method of claim 29 further comprising:

(h) detecting arrhythmic events from the ECG signal; and

(i) discarding the time segment of the natural heart acceleration signal corresponding to the arrhythmic events.

36. The method of claim 25 further comprising:

(g) measuring a heart beat rate; and

(h) varying the length of the selected time segment of the natural heart acceleration signal based on the heart beat rate.

37. The method of claim 36 wherein the length of the time segment of the natural heart acceleration signal is decreased when the heart beat rate increases.

38. The method of claim 36 wherein the length of the time segment of the natural heart acceleration signal is increased when the heart beat rate decreases.

39. The method of claim 25 wherein the therapy delivered to the heart is at least one of electrical pacing pulses, defibrillation, pacer cardioverter defibrillation pulses and drug therapy.

40. An apparatus for providing therapy to a patient'"'"'s heart comprising:

(a) a natural heart acceleration sensor;

(b) means for generating a signal representative of the natural heart acceleration;

(c) means for selecting at least one time segment of the natural heart acceleration signal; and

(d) means for delivering therapy to the heart of the patient based on the selected time segment of the natural heart acceleration signal.

41. The apparatus of claim 40 wherein the at least one time segment is selected in an isovolumetric phase of the patient'"'"'s heart.

42. The apparatus of claim 41 wherein the isovolumetric phase is the isovolumetric contraction phase.

43. The apparatus of claim 41 wherein the isovolumetric phase is the isovolumetric relaxation phase.

44. The apparatus of claim 40 further comprising means for sensing an ECG of the patient and for generating a signal representative of the ECG, and wherein the selection of the at least one time segment of the natural heart acceleration signal is based on a predetermined portion of the ECG signal.

45. The apparatus of claim 44 wherein the predetermined portion of the ECG signal is an isovolumetric phase of the patient'"'"'s heart.

46. The apparatus of claim 45 wherein the isovolumetric phase is the isovolumetric contraction phase.

47. The apparatus of claim 45 wherein the isovolumetric phase is the isovolumetric relaxation phase.

48. The apparatus of claim 44 wherein the predetermined portion of the ECG signal is the QRS.

49. The apparatus of claim 44 wherein the predetermined portion of the ECG signal is the T wave.

50. The apparatus of claim 44 further comprising means for detecting arrhythmic events from the ECG signal, and means coupled to the detecting means for discarding the time segment of the natural heart acceleration signal corresponding to the arrhythmic events.

51. The apparatus of claim 40 further comprising means for measuring a heart beat rate of the patient and means responsive to the heart beat rate for varying the length of the selected time segment of the natural heart acceleration.

52. The apparatus of claim 51 wherein the length of the time segment of the natural heart acceleration signal is decreased when the heart beat rate increases.

53. The apparatus of claim 51 wherein the length of the time segment of the natural heart acceleration signal is increased when the heart beat rate decreases.

54. The apparatus of claim 40 wherein the therapy delivered to the heart is at least one of electrical pacing pulses, defibrillation, pacer cardioverter defibrillation pulses and drug therapy.
