Abstract

A device and method are disclosed for electronically measuring the degree of pulsus paradox in a patient from a single patient date source. The single data source may be any device that outputs a waveform indicatiave of patient pulsatile cardiovascular behavior such as an optical plethysmograph, a pulse oximeter, or a blood pressure monitor. The invention is useful for rapidly assessing the status of a patient in acute respiratory distress for the purpose of objectively determining the severity of their condition.

Claims

1. A device for measuring pulsus paradoxus in a patient, comprising:

an input for receiving data from a sensor indicative of pulsatile cardiovascular behavior of a patient;

a processor for extracting pulse data and respiration data from the pulsatile cardiovascular behavior data wherein said processor employs period amplitude analysis to said pulse data to generate data indicative of the presence of pulsus paradoxus in the patient.

The device of claim 1 wherein the processor determines a maximal pulse peak height and a minimal pulse peak height induced by respiration, and calculates a difference in the maximal pulse peak height and minimal pulse peak height using period amplitude analysis to generate data indicative of pulsus paradoxus.

The device of claim 2 wherein the processor calculates an average difference in the maximal pulse peak height and the minimal pulse peak height induced by patient respiratory data, and compares the average difference in the maximal pulse peak height and the minimal pulse height induced by patient respiratory data, to a difference in maximal pulse peak height and minimal pulse peak height induced by normal patient respiratory data, wherein a disagreement between the calculated average difference in maximal pulse peak height and minimal pulse peak height in the patient, and the calculated difference in the maximal pulse peak height and minimal pulse peak height in the normal respiratory data, is indicative of pulsus paradoxus in the patient.

4. The device of claim 2 wherein the number of calculated differences in maximal pulse peak height and minimal pulse peak height is pre-programmed.

5. The device of claim 1 further comprising a display for displaying the data indicative of pulsus paradoxus.

6. The device of claim 1 wherein the data indicative of pulsatile cardiovascular behavior is obtained from a pulse oximeter.

7. The device of claim 1 wherein the data indicative of pulsatile cardiovascular behavior is obtained from an optical plethysmograph.

8. The device of claim 7 wherein said processor identifies a periodic decrease in plethysmographic amplitude.

9. The device of claim 8 wherein the periodic decreases in plethysmographic amplitude is compared to plethysmographic waveforms stored in memory corresponding to known amounts of pulsus paradoxus.

10. The device of claim 1 wherein the data indicative of pulsatile cardiovascular behavior is obtained from a blood pressure monitor.

11. A method for measuring pulsus paradoxus in a patient comprising the steps of:

obtaining data indicative of pulsatile cardiovascular behavior from a patient;

extracting data indicative of respiration from the pulsatile cardiovascular behavior data;

extracting data indicative of pulse from the pulsatile cardiovascular behavior data;

employing period amplitude analysis to said extracted pulse data and said extracted expiration data; and

generating data indicative of presence of pulsus paradoxus in a patient from said period amplitude analysis.

12. The method of claim 11 further comprising the step of displaying said data indicative of pulsus paradoxus.

13. The method of claim 11 wherein said step of obtaining data indicative of pulsatile cardiovascular behavior from a patient comprises output of a pulse oximeter.

14. The method of claim 11 wherein said step of obtaining data indicative of pulsatile cardiovascular behavior from a patient comprises output of an optical plethysmograph.

15. The method of claim 11 wherein said step of obtaining data indicative of pulsatile cardiovascular behavior from a patient comprises output of a blood pressure monitor.

16. The method of claim 14 further comprising the step of identifying periodic decreases in plethysmographic amplitude.

17. The method of claim 16 further comprising the step of comparing said periodic decreases in plethysmographic amplitude to plethysmographic waveforms stored in memory corresponding to known amounts of pulsus paradoxus.
