Abstract

An implantable medical device such as a pacemaker or implantable cardioverter defibrillator or stand-alone hemodynamic monitor that uses optical plethysmography responsive to variations in arterial pulse amplitude to detect the hemodynamic status of a patient. A light source and light detector are positioned for a reflected-light configuration on a device housing and coupled to electronic circuitry in the housing via a hermetic feedthrough. In one embodiment, the light source and detector are positioned in a recess in a wall of the device housing and surrounded by an encapsulant. The source and detector are positioned for a reflected-light configuration and are preferably an infrared (IR) LED and photodiode, respectively. The source and detector may be placed in a single recess that is created when the device housing is manufactured. An opaque optical barrier is place between the source and detector, which ensures that no light passes between them without first interacting with the overlying tissue. Light reflected from the source to the detector is modulated by pulsation of blood from vasculature in the surrounding body tissue thus enabling sensing of the strength of a heartbeat.

Claims

1. An implantable medical device including a sensor for use in detecting the hemodynamic status of a patient comprising:

a hermetic device housing enclosing device electronics for receiving and processing data; and

said device housing including at least one recess and a sensor positioned in said at least one recess.

2. The implantable medical device of claim 1 wherein said at least one recess includes first and second recesses, said sensor including a light source positioned in said first recess and a light detector positioned in said second recess wherein said light detector receives reflected light originating from said light source and generates a signal which is indicative of variations in the reflected light.

3. The implantable medical device of claim 2 and further including a biocompatible, optically transparent encapsulant filling each of said first and second recesses and surrounding each of said light source and light detector.

The implantable medical device of claim 2 and further including first and second feedthroughs coupled to said light source and said light detector, respectively, said feedthroughs providing a hermetic coupling between said light detector and said light source on the exterior of said device housing and said device electronics contained in said device housing.

5. The implantable medical device of claim 2 wherein said light source is a light emitting diode and said light detector is a photodiode.

The implantable medical device of claim 1 wherein said recess includes an optical barrier and said sensor includes a light source and a light detector positioned on opposite sides of said barrier wherein said light detector receives reflected light originating from said light source and generates a signal which is indicative of variations in the reflected light.

7. The implantable medical device of claim 6 and further including a biocompatible, optically transparent encapsulant said recess and surrounding each of said light source and light detector.

The implantable medical device of claim 6 and further including at least one feedthrough coupled to said light source and to said light detector and providing a hermetic coupling between said light detector and light source on the exterior of said device housing and said device electronics contained in said device housing.

9. The implantable medical device of claim 6 wherein said light source is a light emitting diode and said light detector is a photodiode.

10. An implantable medical device including a hemodynamic sensor for monitoring arterial pulse amplitude comprising:

a device housing;

a transducer comprising a light source and a light detector positioned exterior to said device housing responsive to variations in arterial pulse amplitude; and

wherein said light detector receives light originating from said light source and reflected from arterial vasculature of a patient and generates a signal which is indicative of variations in the reflected light caused by the expansion and contraction of said arterial vasculature.

11. The implantable medical device of claim 10 wherein said light source emits a single wavelength of light.

12. The implantable medical device of claim 10 and further including an optical shield positioned between said light source and said light detector to prevent direct transmission of light from said source to said detector.

13. The implantable medical device of claim 10 wherein said light source and said light detector are positioned relative to each other to prevent direct transmission of light from said source to said detector.

14. An implantable medical device including a hemodynamic sensor for monitoring arterial pulse amplitude comprising:

a device housing; and

an ultrasound transducer associated with said device housing responsive to variations in arterial pulse amplitude.

15. An implantable medical device including a hemodynamic sensor for monitoring arterial pulse amplitude comprising:

a device housing; and

a transducer associated with said device housing responsive to variations in arterial pulse amplitude,

said device housing having at least one substantially planar face and said transducer is positioned on said planar face.

16. The implantable medical device of claim 15 and further including a biocompatible encapsulant covering said transducer.

17. An implantable medical device comprising:

a vascular plethysmography sensor configured for extravascular placement; and

electronic circuitry for controlling said medical device and coupled to receive sensed plethysmography signals from said sensor and for processing said sensed plethysmography signals and control said device in response thereto,

wherein said medical device is an implantable pulse generator for providing pacing therapy to a patient'"'"'s heart and wherein said electronic circuitry uses said sensed plethysmography signals to verify that said pacing therapy was successfully delivered.

18. An implantable medical device comprising:

a vascular plethysmography sensor configured for extravascular placement; and

electronic circuitry for controlling said medical device and coupled to receive sensed plethysmography signals from said sensor and for processing said sensed plethysmography signals and control said device in response thereto,

wherein said medical device further includes a sensing lead for coupling to said patient'"'"'s heart to sense electrical signals from the heart and wherein said electronic circuitry uses said sensed plethysmography signals to optimize electrical sensing with said sensing lead.

19. An implantable medical device comprising:

a vascular plethysmography sensor configured for extravascular placement; and

electronic circuitry for controlling said medical device and coupled to receive sensed plethysmography signals from said sensor and for processing said sensed plethysmography signals and control said device in response thereto,

wherein said vascular plethysmography sensor comprises a light source and a light detector and wherein vascular plethysmography is performed by using said light detector to sense variations in reflected light originating in said light source and directed at a patient'"'"'s arterial vasculature.

20. An implantable pulse generator for providing pacing therapy to a patient'"'"'s heart comprising:

a vascular plethysmography sensor configured for extravascular placement; and

electronic circuitry for controlling said pulse generator and coupled to receive sensed plethysmography signals from said sensor to optimize the timing of pacing pulses provided to said patient'"'"'s heart.

21. The medical device of claim 20 and further including a posture sensor and wherein the timing of pacing pulses is optimized upon sensing of a change of the patient'"'"'s posture by said posture sensor.

22. An implantable medical device comprising:

a vascular plethysmography sensor configured for extravascular placement; and

electronic circuitry for controlling said medical device and coupled to receive sensed plethysmography signals from said sensor and for processing said sensed plethysmography signals to discriminate among possible cardiac arrhythmias and control said device in response thereto.

23. An implantable medical device comprising:

a vascular plethysmography sensor configured for extravascular placement;

an accelerometer; and

electronic circuitry for controlling said medical device and coupled to receive sensed plethysmography signals from said sensor and for processing said sensed plethysmography signals and controlling said device in response thereto, wherein said accelerometer detects motion artifacts that could interfere with said sensed plethysmography signals.

24. An implantable pulse generator for providing pacing therapy to a patient'"'"'s heart comprising:

a vascular plethysmography sensor configured for extravascular placement; and

electronic circuitry for controlling said pulse generator and coupled to receive sensed plethysmography signals from said sensor to optimize the energy level of pacing pulses provided to said patient'"'"'s heart.
