1. A diabetes care management system for managing blood glucose levels associated with diabetes, comprising:
- a computing device, comprising;
a memory configured to store (i) a plurality of optimal blood glucose values at a plurality of times, (ii) a plurality of measured blood glucose values of a patient at said times, (iii) a plurality of insulin values at said times, and (iv) a plurality of optimal insulin values at said times;
a microprocessor, in communication with said memory, said microprocessor programmed to (A) generate a plurality of scaling factors customized to said patient each in response to a change in any one of said insulin values or said measured blood glucose values at any one of said times and (B) calculate a future blood glucose value based on (i) a plurality of differentials, one of said differentials calculated as one of said scaling factors multiplied by differences between one of said insulin values and one of said optimal insulin values, (ii) a sum of said differentials plus (iii) a difference between one of said measured blood glucose values and a corresponding one of said optimal blood glucose values; and
a display configured to display said future blood glucose value; and
an insulin delivery device configured to deliver insulin in response to said future blood glucose value, wherein the system is configured to monitor and control the blood glucose levels on a regular basis by the patient outside of a medical clinic.

Abstract
A diabetes care management system for managing blood glucose levels associated with diabetes comprising a computing device and an insulin delivery device. The computing device generally includes (i) a memory comprising one or more optimal blood glucose values, one or more self care values of a patient, one or more measured blood glucose values, and one or more scaling factors for weighting the impact on a future blood glucose value and that are customizable to an individual patient to predict the effect on the blood glucose of self care actions performed by the individual patient; (ii) a microprocessor, in communication with the memory, programmed to (A) determine the one or more scaling factors from one or more physiological parameters including body mass, metabolism rate, fitness level or hepatic or peripheral insulin sensitivity, or combinations thereof, and (B) calculate a further value, the further value being based on the self care values, and on the one or more optimal blood glucose values, and on the one or more scaling factors; and (iii) a display configured to display information according to the further value; and (iv) a housing, wherein the memory and the microprocessor are housed within the housing, thereby providing a hand-held, readily transportable computing device. The insulin delivery device may deliver insulin in response to information associated with the further value.

15. A diabetes care management system for use in managing a physiological condition of a continuous nature associated with glucose levels and diabetes care, comprising:

a computing device, comprising; a memory configured to store (i) a plurality of optimal control parameter values at a plurality of times, (ii) a plurality of self care values of a patient at said times, (iii) a plurality of measured control parameter values at said times, and (iv) a plurality of optimal self care values at said times; and a microprocessor, in communication with said memory, said microprocessor programmed to (A) generate a plurality of scaling factors customized to said patient each in response to a change in any one of said self care values or said measured control parameter values at any one of said times and (B) calculate a future control parameter value based on (i) a plurality of differentials, one of said differentials calculated as one of said scaling factors multiplied by differences between one of said self care values and one of said optimal self care values, (ii) a sum of said differentials plus, (iii) a difference between one of said measured control parameter values and a corresponding one of said optimal control parameter values; and a housing, wherein said memory and said microprocessor are housed within said housing, thereby providing a hand-held, transportable computing device; and an insulin delivery device configured to deliver insulin in response to said future control parameter value, wherein because of the continuous nature of the physiological condition associated with the glucose levels and the diabetes care, the system is configured to monitor and control the measured control parameter values on a regular basis by the patient outside of a medical clinic. - View Dependent Claims (16, 17, 18, 19, 20, 21, 22, 23)
