Abstract

An implantable medical device for delivering a therapeutic substance to a delivery site in a patient. A reservoir holds a supply of the fluid therapeutic substance. A catheter has a proximal end, a delivery region and a lumen extending from the proximal end to the delivery region. The proximal end of the catheter is operatively coupled to the reservoir. The delivery region of the catheter is adapted to be placed proximate the delivery site in the patient. The therapeutic substance is adapted to be delivered through the lumen to the patient. A sensing device is operatively coupled with the lumen of the catheter being capable of detecting a pressure of the therapeutic substance in the lumen. A controller is operatively coupled to the sensing device, the controller being capable of taking an action in response to the pressure in the lumen.

Claims

1. A method of delivering a therapeutic substance to a delivery site in a patient, comprising the steps of:

pumping said therapeutic substance under pressure from a reservoir of an implantable medical device implanted at a first location of a patient through a catheter fluidly coupled to said reservoir, said catheter having a proximal end coupled to the implantable medical device, a delivery region and a first lumen extending between said proximal end and said delivery region, said delivery region of said catheter placed in proximity to said delivery site in said patient, wherein the delivery site is removed from the first location of the patient;

detecting a pressure of said therapeutic substance in said first lumen;

detecting a reference pressure in the patient in proximity to the delivery region of said catheter;

comparing the pressure in said first lumen to the reference pressure to obtain a relative pressure in said first lumen; and

taking an action in response to said relative pressure in said lumen.

2. A method as in claim 1 wherein said therapeutic substance comprises a fluid.

3. A method as in claim 2 wherein said therapeutic substance comprises a liquid.

4. A method as in claim 1 wherein said action is taken when said pressure exceeds a predetermined level.

5. A method as in claim 4 wherein said action is based upon an obstruction in said lumen.

6. A method as in claim 1 wherein said lumen has a restriction and wherein said action is taken when said pressure drops below a predetermined level.

7. A method as in claim 6 wherein said action is based upon a leak of said catheter.

8. A method as in claim 1 wherein said action is taken when said pressure has a characteristic signature.

9. A method as in claim 8 wherein said characteristic signature comprises a transient rise in said delivery rate of said therapeutic substance.

10. A method as in claim 8 wherein said characteristic signature comprises a transient fall in said delivery rate of said therapeutic substance.

11. A method as in claim 8 wherein said characteristic signature comprises a transient rise and subsequent fall in said delivery rate of said therapeutic substance.

12. A method as in claim 11 wherein said characteristic signature is a function of a decay time of said pressure following said fall of said delivery rate of said therapeutic substance.

13. A method as in claim 8 wherein said pumping said therapeutic substance comprises pumping via a plurality of rollers of a peristaltic pump having a tubular-structure, and wherein said characteristic signature comprises said pressure dropping below a predetermined threshold in combination with a lack of a pressure dip created each time one of said plurality of rollers lifts off of said tubular-structure in said peristaltic pump.

14. A method as in claim 1 wherein said action comprises ceasing delivery of said therapeutic substance to said patient.

22. A method as in claim 1, wherein the catheter comprises a second lumen and wherein detecting the reference pressure comprises detecting pressure in the second lumen.

23. A method as in claim 22, wherein comparing the pressure in said first lumen to the reference pressure comprises comparing the pressures via a differentinal pressure sensor.

24. A method as in claim 23, wherein the differential pressure sensor comprises a diaphragm and wherein comparing the pressures comprises detecting deflection of the diaphragm.

15. A method of delivering a therapeutic substance to a delivery site in a patient comprising the steps of:

pumping said therapeutic substance under pressure from a reservoir of an implantable medical device implanted at a first location of a patient through a catheter fluidly coupled to said reservoir, said catheter having a proximal end coupled to the implantable medical device, a delivery region and a lumen extending between said proximal end and said delivery region, said delivery region of said catheter placed in proximity to said delivery site in said patient, wherein the delivery site is removed from the first location of the patient;

detecting pressure of said therapeutic substance in said lumen;

determining whether said pressure has a characteristic signature in response to a transient change in flow comprising a rise and subsequent fall in delivery rate of the therapeutic substance as a result of pulsed delivery of said therapeutic substance; and

taking action in response to said pressure in said lumen when said pressure has said characteristic signature.

A method as in claim 15 further comprising establishing a baseline decay time for said characteristic pressure signature when said characteristic pressure signature is a function of decay time of pressure, wherein the baseline decay time established is for decay time for the normal delivery rate of the therapeutic substance.

17. A method as in claim 16 wherein said action is based on a reduction in the decay time of the characteristic pressure signature relative to the baseline decay time.

18. A method as in claim 17 wherein the action comprises estimating a location wherein the catheter may be leaking or broken by measuring the amount of decrease in the decay time of the characteristic pressure signature relative to the baseline decay time.

19. A method as in claim 16 wherein said action is based upon increase in the decay time of the characteristic pressure signature relative to the baseline decay time.

20. A method as in claim 15 wherein said characteristic pressure signature is a function of decay time of pressure and the action taken is based upon a lack of pressure decay occurring within a predetermined time period.

21. A method as in claim 20 wherein the action comprises determining the location of an obstruction in the catheter by measuring the amplitude of a step change in pressure resulting from the lack of pressure decay.
