Abstract

Devices, systems and methods for stenting, spacing, draining, ventilating and/or delivering drugs and other therapeutic or diagnostic substances to desired locations within the bodies of human or non-human animal subjects, including methods and systems for treating paranasal sinusitis and ethmoid disease.

Claims

1. A system for treating ethmoid disease in a human or animal subject, said system comprising:

a penetrator comprising a rigid member having a sharp distal tip, the rigid member being advanceable into the nose and the sharp distal tip being insertable through the ethmoid bulla to form an opening into at least one ethmoid air cell;

a penetrator sheath having a proximal imagable marker and a distal imagable marker, said penetrator sheath being positionable over the penetrator as the penetrator is being advanced to form said opening, said penetrator being thereafter removeable leaving the penetrator sheath within the formed opening; and

an implantable device comprising a substance delivery reservoir, a tube through which a substance is introduceable into the reservoir to inflate the reservoir and a backflow prevention element that is operative to allow a substance to be infused in a first direction through the tube and into the reservoir but prevents substance that has entered the reservoir from backflowing in a second direction through the tube, said implantable device being insertable, with the reservoir in a deflated configuration, into the penetrator sheath after removal of the penetrator therefrom, and being advanceable to a fully-advanced position at which further advancement is prevented;

the proximal and distal imagable markers of the penetrator sheath being spaced apart and positioned on the sheath so that they provide predictive indications of the locations at which the proximal and distal ends of the reservoir will reside within the sheath when the implantable device has been inserted to its fully-advanced position within the penetrator sheath;

the reservoir being constructed such that, when inflated, the substance will pass out of the reservoir over a period of time causing the configuration of the reservoir to transition toward the deflated configuration.

A system according to claim 1, wherein the implantable device further comprises an elongate shaft having a proximal end, a distal end and a lumen, said reservoir being positioned on the tubular shaft in communication with the lumen such that an inflation substance may be introduced through the lumen into the reservoir.

3. A system according to claim 1, wherein the implantable device acts as a spacer when implanted, thereby maintaining a space in the anatomy.

4. A system according to claim 1, wherein at least part of the reservoir is permeable such that a diagnostic or therapeutic substance may elute out of the reservoir while the device is implanted.

5. A system according to claim 1, wherein the implantable device further comprises:

at least one position maintaining member that abuts, engages or attaches to an adjacent anatomical structure to substantially maintain the reservoir at said location within the subject'"'"'s body.

6. A system according to claim 5, wherein the position maintaining member comprises at least one projection that frictionally engages an adjacent anatomical structure so as to deter migration or passage of the reservoir back out of the opening.

A system according to claim 6, wherein said at least one projection is positionable in a collapsed position during insertion through the opening and is subsequently transitionable to an extended position wherein said at least one projection frictionally engages at least one adjacent anatomical structure.

8. A system according to claim 7, wherein said at least one projection is biased to said extended configuration.

9. A system according to claim 8, wherein said at least one projection is formed substantially of nickel-titanium alloy.

10. A system according to claim 8, wherein the implantable device is removable by pulling the device with enough force to overcome the bias of said at least one projection thereby allowing the implantable device to be removed.

11. A system according to claim 1, wherein the penetrator comprises an elongate member having a sharp distal tip and an angle formed therein.

12. A system according to claim 11, wherein the angle is from about 0 degrees to about 70 degrees.

13. A system according to claim 1, wherein at least a portion of the tube is removable following inflation of the reservoir.

14. A system according to claim 13, wherein the backflow prevention element comprises a one way a valve.

15. A system according to claim 1, wherein the reservoir comprises multiple apertures and the substance passes out of the reservoir through said apertures.

16. A system according to claim 15, wherein said apertures are approximately 5 μ

m to approximately 80 μ

m in diameter.

17. A system according to claim 15, wherein said apertures are approximately 50 to approximately 5000 in number and approximately 5 μ

m to approximately 80 μ

m in diameter.

18. A system according to claim 1, wherein the inflatable reservoir has a diameter of about 3.5 mm and a length of about 13 mm.

19. A system according to claim 1, wherein the implantable device further comprises an iontophoresis electrode useable to effect iontophoretic delivery of the substance.

20. A system according to claim 19, further comprising a power source connectable to the iontophoresis electrode.

21. A system according to claim 1 wherein the penetrator sheath comprises proximal sheath portion, a distal sheath portion and an abutment surface therebetween.

A system according to claim 21 wherein the implantable device is advanceable, with the constraining member positioned in its first position, into the penetrator sheath after the penetrator has been removed therefrom, thereby causing i) the constraining member to abut against the abutment surface, ii) the constraining member to move proximally to its second position and iii) a distal portion of the implantable device, including the collapsed reservoir and said at least one projection, to advance into the distal sheath portion.

A system according to claim 22 wherein, after the implantable device has been advanced into the penetrator sheath, the penetrator sheath and the constraining member may be withdrawn proximally, thereby exposing the distal portion of the implantable device and allowing said at least one projection to transition to its extended position.

24. A system according to claim 23 wherein the implantable device further comprises a suture receiving member and wherein proximally withdrawing the penetrator sheath and the constraining member also exposes the suture receiving member such that it may be sutured to tissue within the nasal cavity.
