Abstract

A device for removing tissue from a patient may include an elongate flexible body having a proximal end, a distal end, and a longitudinal axis therebetween, the elongate body having opposed first and second major surfaces with a lateral orientation across the axis, and a plurality of blades distributed laterally across the first major surface. Each blade may have a first end adjacent the first surface and extending to a cantilevered second end, a first edge between the first and second ends of the blade being oriented toward the distal end of the elongate body, a second edge between the first and second ends of the blade being oriented toward the proximal end of the elongate body, a height of the blade between its first and second ends, and an axial length of the blade between its first and second edges.

Claims

1. A device for removing tissue from a patient, the device comprising:

an elongate body that is flexible over at least a portion of its length, the elongate body having a proximal end, a distal end, and a longitudinal axis therebetween, the elongate body having opposed first and second major surfaces with a lateral orientation across the axis, the elongate body further comprising a rigid shaft, a flexible portion extending from one end of the shaft, a guidewire coupler on or in the flexible portion, and a first handle coupled with an end of the shaft opposite the flexible portion; and

a plurality of blades on the first major surface, each blade having a first end adjacent the first major surface and extending to a cantilevered second end, a first edge between the first and second ends of the blade being oriented toward the distal end of the elongate body, a second edge between the first and second ends of the blade being oriented toward the proximal end of the elongate body, a height of the blade between its first and second ends, and an axial length of the blade between its first and second edges,

wherein at least one of the first edge or the second edge comprises a cutting edge so as to cut the tissue when the first major surface is urged toward the tissue and the elongate body advances along a path toward one end of the elongate body,

wherein both the height and the axial length of each blade are greater than a transverse width of the blade, and

wherein the guidewire coupler is configured for attachment to the guidewire from the distal direction of the distal end, and wherein the guidewire extends distal of the distal end when the guidewire is coupled to the guidewire coupler.

2. A device as in claim 1, wherein each blade has an associated base extending along and affixed to the first major surface.

3. A device as in claim 2, wherein at least some of the bases are disposed laterally between a first associated blade and a second associated blade.

4. A device as in claim 1, wherein both the first edge and the second edge of each blade comprises a cutting edge so as to cut the tissue and effect removal of the tissue when the elongate body reciprocates along the path.

5. A device as in claim 1, wherein a frontal surface area of the first or second edge of each blade is less than a side surface area of each blade.

A device as in claim 1, wherein a side of each blade between its two edges forms an angle with the first major surface of the elongate body of between 45 degrees and 90 degrees, and wherein the side of each blade is aligned at an angle of between 0 degrees and 45 degrees relative to the longitudinal axis of the elongate body.

7. A device as in claim 6, wherein the side of each blade forms an angle with the first major surface of between 60 degrees and 90 degrees, and wherein the side of each blade is aligned at an angle of between 0 degrees and 30 degrees relative to the longitudinal axis of the elongate body.

8. A device as in claim 6, wherein at least two blades are aligned at different angles relative to the longitudinal axis of the elongate body.

9. A device as in claim 1, wherein the elongate body is configured to bend over a curved surface.

10. A device as in claim 9, wherein at least some of the blades are axially separated from one another along the longitudinal axis of the elongate body so as to allow axial bending of the substrate therebetween.

A device as in claim 9, wherein the device is configured to modify spinal tissue, and wherein the elongate body is configured to extend into the patient'"'"'s body, along a curved path through an intervertebral foramen of the spine, and out of the patient'"'"'s body, such that a flexible portion of the elongate body of the device extends through the intervertebral foramen.

12. A device as in claim 11, wherein a height of at least one blade is at least equal to a thickness of a ligamentum flavum of the spine.

14. A device as in claim 1, wherein the second end of each blade has a shape selected from the group consisting of a pointed tip, a flat edge, a round edge, a serrated edge, a saw-toothed edge and a curved edge.

15. A device as in claim 14, wherein second ends of at least two blades have different shapes, relative to one another.

16. A device as in claim 1, wherein at least two blades have different heights, relative to one another.

17. A device as in claim 1, wherein the blades are fixedly attached to the first major surface.

13. A device as in claim l, wherein the device further includes:

a guidewire configured to couple with the guidewire coupler; and

a second handle configured to couple with the guidewire outside the patient.

18. A device for removing tissue from a patient, the device comprising:

an elongate body that is flexible over at least a portion of its length, the elongate body having a proximal end, a distal end, and a longitudinal axis therebetween, the elongate body having opposed first and second major surfaces with a lateral orientation across the axis, the elongate body further comprising a rigid shaft, a flexible portion extending from one end of the shaft, a guidewire coupler on or in the flexible portion, and a first handle coupled with an end of the shaft opposite the flexible portion; and

a plurality of blades distributed laterally across the first major surface, each blade having a first end adjacent the first surface and extending from the first major surface,

wherein each blade is substantially in-line with the longitudinal axis of the elongate body,

wherein each blade is substantially vertical relative to the first major surface, and

wherein the guidewire coupler is configured for attachment to the guidewire from the distal direction of the distal end, and wherein the guidewire extends distal of the distal end when the guidewire is coupled to the guidewire coupler.

19. A device as in claim 18, wherein a side of each blade is aligned at an angle of between 0 degrees and 30 degrees relative to the longitudinal axis of the elongate body, and wherein the side of each blade forms an angle with the first major surface of between 60 degrees and 90 degrees.

20. A device as in claim 18, wherein each blade has an associated base extending along and affixed to the first major surface.

21. A device as in claim 20, wherein at least some of the bases are disposed laterally between a first associated blade and a second associated blade.

22. A device as in claim 18, wherein both the first edge and the second edge of each blade comprises a cutting edge so as to axially cut the tissue and effect removal of the tissue when the elongate body reciprocates along the path.

23. A device as in claim 18, wherein a frontal surface area of the first or second edge of each blade is less than a side surface area of each blade.

24. A device as in claim 18, wherein a side of each blade between its two edges forms an angle with the first major surface of the elongate body of between 45 degrees and 90 degrees, and wherein the side of each blade is aligned at an angle of between 0 degrees and 45 degrees relative to the longitudinal axis of the elongate body.

25. A device as in claim 24, wherein the side of each blade forms an angle with the first major surface of between 60 degrees and 90 degrees, and wherein the side of each blade is aligned at an angle of between 0 degrees and 30 degrees relative to the longitudinal axis of the elongate body.

26. A device as in claim 24, wherein at least two blades are aligned at different angles relative to the longitudinal axis of the elongate body.

27. A device as in claim 18, wherein the elongate body is configured to bend over a curved surface.

28. A device as in claim 27, wherein at least some of the blades are axially separated from one another along the longitudinal axis of the elongate body so as to allow axial bending of the substrate therebetween.

29. A device as in claim 27, wherein the device is configured to modify spinal tissue, and wherein the elongate body is configured to extend into the patient'"'"'s body, along a curved path through an intervertebral foramen of the spine, and out of the patient'"'"'s body, such that a flexible portion of the elongate body of the device extends through the intervertebral foramen.

30. A device as in claim 29, wherein a height of at least one blade is at least equal to a thickness of a ligamentum flavum of the spine.

31. A device as in claim 18, wherein the device further includes:

a guidewire configured to couple with the guidewire coupler; and

a second handle configured to couple with the guidewire outside the patient.

32. A device as in claim 18, wherein the second end of each blade has a shape selected from the group consisting of a pointed tip, a flat edge, a round edge, a serrated edge, a saw-toothed edge and a curved edge.

33. A device as in claim 32, wherein second ends of at least two blades have different shapes, relative to one another.

34. A device as in claim 18, wherein at least two blades have different heights, relative to one another.

35. A device as in claim 18, wherein the blades are fixedly attached to the first major surface.
