Abstract

An apparatus and a method are provided for treating a targeted area of ocular tissue in a tissue-sparing manner comprising use of two or more therapeutic modalities, including thermal radiation source (such as an CW infrared fiber laser), operative in a wavelength range that has a high absorption in water, and photochemical collagen cross-linking (CXL), together with one or more specific system improvements, such as peri-operative feedback measurements for tailoring of the therapeutic modalities, an ocular tissue surface thermal control/cooling mechanism and a source of deuterated water/riboflavin solution in a delivery system targeting ocular tissue in the presence of the ultraviolet radiation. Additional methods of rapid cross-linking (RXL), are provided that further enables cross-linking (CXL) therapy to be combined with thermal therapy.

Claims

1. An apparatus for treatment of ocular tissue comprising:

a laser operative at a wavelength selected from the ranges of 1.4 μ

m to 1.54μ

m or 1.8 μ

m to 6.0 μ

m for irradiating an ocular surface and inducing symmetrically uniform lesions in a subsurface region below the ocular surface to modify the shape of the ocular surface;

an ocular surface cooling mechanism configured for controlling temperature of the ocular surface irradiated by the laser; and

an ultraviolet radiation source configured to irradiate the ocular surface and the subsurface region to induce crosslinking of ocular tissue below the ocular surface in or near the lesions.

2. The apparatus of claim 1, further comprising a solution source for delivery below the surface of the ocular surface, wherein the solution promotes crosslinking of ocular tissue.

3. The apparatus of claim 2, wherein the solution is deuterated water riboflavin solution.

4. The apparatus of claim 1, wherein the laser is a continuous wave infrared fiber laser.

5. The apparatus of claim 1, wherein the cooling mechanism comprises a sapphire lens configured to applanate on the ocular surface.

6. The apparatus of claim 1, wherein the cooling mechanism comprises a lens made of a highly thermally conductive material configured to applanate on the ocular surface.

7. The apparatus of claim 1, wherein the cooling mechanism comprises a water channel to transport cooled water for controlling a temperature of a lens configured to applanate on the ocular surface.

8. The apparatus of claim 1, wherein the ocular surface is a cornea surface and the subsurface region comprises the stroma.

9. The apparatus of claim 1, wherein the laser wavelength selected from the range of 1.4 μ

m to 1.54 μ

m.

10. The apparatus of claim 1, wherein the laser wavelength is selected from the range of from 1.86 μ

m to 2.52 μ

m.
