1. An access port for providing subcutaneous access to a patient, comprising:

  • a port base defining a fluid cavity, the port base formed from a base material;

    a substrate separate from the port base formed from a substrate material, the substrate attached to the port base, the substrate including an inner surface facing toward the fluid cavity and an outer surface facing away from the fluid cavity; and

    a radiopaque material adhered to the substrate inner surface prior to attachment of the substrate to the port base, the radiopaque material including an identification feature.
