26. An integrated system for monitoring a glucose concentration in a host and for delivering insulin to a host, comprising:

a continuous glucose sensor, wherein the continuous glucose sensor is configured to substantially continuously measure a glucose concentration in a host, and to provide sensor data associated with the glucose concentration in the host; an electronics module comprising; a basal controller module configured to iteratively determine a basal insulin therapy instruction in response to an evaluation of a relationship of internally derived data and a basal profile, wherein the basal profile comprises at least one time block associated with a maximum insulin delivery rate; a bolus controller module configured to iteratively determine a bolus insulin therapy instruction in response to an evaluation of a relationship of internally derived data and an engageable bolus constraint, wherein a relationship of internally derived data to the bolus constraint is evaluated in response to engagement of the bolus constraint, and wherein the bolus constraint comprises a maximum total insulin dose that can be delivered within a predefined time period in response to engagement of the bolus constraint; an on/off controller module configured to iteratively determine an on/off insulin therapy instruction in response to an evaluation of a relationship of internally derived data and a glucose boundary, wherein the insulin therapy instruction comprises an instruction selected from the group consisting of on and off; and an insulin delivery device configured to deliver insulin to the host, wherein the insulin delivery device is at least one of physically connected to a receiver and operably connected to a receiver, wherein the insulin delivery device is configured to receive the insulin therapy instruction from the basal controller module, the bolus controller and/or the on/off controller module. - View Dependent Claims (27, 28, 29, 30, 31, 32, 33, 34, 35, 36, 37)

38. An method for monitoring a glucose concentration in a host and for delivering insulin to a host, comprising:

substantially continuously receiving sensor data from a continuous glucose sensor indicative of the glucose concentration in the host; iteratively determining, using a basal controller module, a basal insulin therapy instruction in response to an evaluation of a relationship of internally derived data and a basal profile, wherein the basal profile comprises at least one time block associated with a maximum insulin delivery rate; iteratively determining, using a bolus controller module, a bolus insulin therapy instruction in response to an evaluation of a relationship of internally derived data and an engageable bolus constraint, wherein a relationship of internally derived data to the bolus constraint is evaluated in response to engagement of the bolus constraint, and wherein the bolus constraint comprises a maximum total insulin dose that can be delivered within a predefined time period in response to engagement of the bolus constraint; iteratively determining, using an on/off controller module, an on/off insulin therapy instruction in response to an evaluation of a relationship of internally derived data and a glucose boundary, wherein the insulin therapy instruction comprises an instruction selected from the group consisting of on and off; and receiving the insulin therapy instruction from the basal controller module, the bolus controller and/or the on/off controller module and delivering insulin to the host, using an insulin delivery device.
