Abstract

Methods, systems, and apparatus for Near Infrared Microbial Elimination Laser Systems (NIMELS) including use with medical devices are disclosed. The medical devices can be situated in vivo. Suitable medical devices include catheters, stents, artificial joints, and the like. NIMELS methods, systems, and apparatus can apply near infrared radiant energy of certain wavelengths and dosimetries capable of impairing biological contaminants without intolerable risks and/or adverse effects to biological moieties other than a targeted biological contaminant associated with traditional approaches described in the art (e.g., loss of viability, or thermolysis). Lasers including diode lasers may be used for one or more light sources. A delivery assembly can be used to deliver the optical radiation produced by the source(s) produced to an application region that can include patient tissue. Exemplary embodiments utilize light in a range of 850 nm-900 nm and/or 905 nm-945 nm at suitable NIMELS dosimetries.

Claims

1. A method of reducing the level of a biological contaminant in a target site without an intolerable adverse effect on a biological moiety, comprising the step of irradiating the target site with an optical radiation having a wavelength from about 905 nm to about 945 nm at a NIMELS dosimetry.

5. The method of claim 1, wherein the biological contaminant is selected from the group consisting of bacteria, fungi, molds, mycoplasmas, protozoa, prions, parasites, and viruses.

6. The method of claim 1, wherein the biological contaminant is a selected from the group consisting of Trichophyton, Microsporum, Epidermophyton, Candida, Scopulariopsis brevicaulis, Fursarium spp., Aspergillus spp., Alternaria, Acremonium, Scytalidinum, dimidiatum, and Scytalidinium hyalinum.

7. The method of claim 1, wherein the biological contaminant is Trichophyton.

8. The method of claim 1, wherein the biological contaminant is E. coli.

9. The method of claim 1, wherein the biological contaminant is Staphylococcus.

10. The method of claim 1, wherein the biological contaminant is Candida.

14. The method of claim 1, wherein the optical radiation is provided for a time (Tn) of from about 50 to about 300 seconds.

15. The method of claim 1, wherein the optical radiation is provided for a time (Tn) of from about 75 to about 200 seconds.

16. The method of claim 15, wherein the optical radiation is provided for a time (Tn) of from about 100 to about 150 seconds.

17. The method of claim 1, wherein the optical radiation is provided for a time (Tn) of from about 100 to about 450 seconds.

18. The method according to claim 1, wherein the NIMELS dosimetry provides an energy density from about 100 J/cm² to about 500 J/cm₂.

19. The method according to claim 18, wherein the NIMELS dosimetry provides an energy density from about 175 J/cm² to about 300 J/cm².

20. The method according to claim 19, wherein the NIMELS dosimetry provides an energy density from about 200 J/cm² to about 250 J/cm².

21. The method according claim 1, wherein the NIMELS dosimetry provides an energy density from about 300 J/cm² to about 700 J/cm².

22. The method according to claim 21, wherein the NIMELS dosimetry provides an energy density from about 300 J/cm² to about 500 J/cm².

23. The method according to claim 22, wherein the NIMELS dosimetry provides an energy density from about 300 J/cm² to about 450 J/cm².

24. The method according to claim 1, wherein the target site includes a medical device.

25. The method according to claim 24, further comprising delivering the optical radiation to the target site by one or more optical fibers.

26. The method according to claim 24, further comprising delivering the optical radiation to the target site by a hollow wave guide.

27. The method according to claim 24, further comprising delivering the optical radiation to the target site by free beam transmission.

2. A method of reducing the level of a biological contaminant in a target site without an intolerable adverse effect on a biological moiety, comprising the step of irradiating the target site with an optical radiation having from about 925 nm to about 935 nm at a NIMELS dosimetry.

3. A method of reducing the level of a biological contaminant in a target site without an adverse effect on a biological moiety, comprising the steps of:

(a) irradiating the target site with an optical radiation having a wavelength from about 850 nm to 900 nm at a NIMELS dosimetry; and

(b) irradiating the target site with a second optical radiation having a wavelength 905 nm to about 950 nm.

11. The method of claim 3, wherein steps (a) and (b) are performed independently.

12. The method of claim 3, wherein steps (a) and (b) are performed in sequence.

13. The method of claim 3, wherein steps (a) and (b) are performed essentially concurrently.

4. A method of reducing the level of a biological contaminant in a target site without an adverse effect on a biological moiety, comprising the steps of:

(a) irradiating the target site with an optical radiation having a wavelength from about 865 nm to 875 nm at a NIMELS dosimetry; and

(b) irradiating the target site with a second optical radiation having a wavelength 925 nm to about 935 nm.

28. A therapeutic system comprising:

an optical radiation generation device configured and arranged to generate optical radiation substantially in a first wavelength range from about 850 nm to about 900 nm or a second optical radiation range having a wavelength from about 905 nm to about 950 nm, or both wavelength ranges;

a delivery assembly for causing the optical radiation to be transmitted through an application region; and

a controller operatively connected to the optical radiation generation device for controlling dosage of the radiation transmitted through the application region at a NIMELS dosimetry, whereby the time integral of the power density of the transmitted radiation per unit area is below a predetermined threshold of a NIMELS dosimetry.

29. A therapeutic system according to claim 28, wherein the optical radiation source includes at least one laser configured and arranged to produce the radiation in the first wavelength range, or the second wavelength range, or both ranges, and at the NIMELS dosimetry.

30. A therapeutic system according to claim 29, wherein the at least one laser includes a diode laser configured and arranged to produce an output in the near infrared region.

A therapeutic system according to claim 29, wherein the diode laser includes a semiconductor material selected from the group consisting of In_xGa_1-xAs, GaAs_1-xP_x, Al_xGa_1-xAs, and (Al_xGa_1-x)_yIn_1-yAs, for producing radiation in the first wavelength range or the second wavelength range or both ranges.

32. A therapeutic system according to claim 28, wherein the optical radiation has a desired degree of coherence.

33. A therapeutic system according to claim 28, wherein the controller is configured and arranged to control the radiation to be a succession of radiation pulses.

34. A therapeutic system according to claim 28, wherein the controller is further configured and arranged to control the intensity of the radiation pulses.

35. A therapeutic system according to claim 28, wherein the controller is further configured and arranged to control the temporal width of the radiation pulses.

36. A therapeutic system according to claim 28, wherein the controller is further adapted to control the repetition rate of the radiation pulses.

37. A therapeutic system according to claim 36, wherein the controller is further configured to control the intensity of the radiation pulses.

38. A therapeutic system according to claim 36, wherein the controller is further configured and arranged to control the temporal width of the radiation pulses.

39. A therapeutic system according to claim 28, wherein the controller is preprogrammed to control the optical radiation generation device to deliver a predetermined dose of the radiation by way of the application surface to a treatment site.

40. A therapeutic system according to claim 28, wherein the controller comprises a dosimetry calculator which is preprogrammed to calculate dosage needed for treatment of the treatment site based on anatomic data of the treatment site.

41. A therapeutic system according to claim 40, wherein the dosimetry calculator further comprises an imaging system for imaging the treatment site and generating the anatomic data of the treatment site.

42. A therapeutic system according to claim 40, wherein the treatment site is a lesion and the anatomic data includes size, type, and depth of the lesion.

43. A therapeutic system according to claim 28, wherein the optical radiation is selected to generate reactive oxygen in tissue of a patient (in vivo) at a treatment site upon which the optical radiation is incident.

44. A therapeutic system according to claim 28, wherein the delivery assembly comprises one or more optical fibers configured and arranged to receive radiation from the optical radiation generation device and deliver the radiation to a medical device disposed in tissue of a patient (in vivo).

45. A therapeutic system according to claim 44, wherein the one or more optical fibers include a plurality of optical fibers having an end configured and arranged for insertion in patient tissue at a location within optical transmission range of the medical device, wherein the radiation can be delivered at a NIMELS dosimetry to the tissue surrounding the medical device.

46. A therapeutic system according to claim 44, wherein the medical device is a stent.

47. A therapeutic system according to claim 44, wherein the medical device is an artificial joint.

48. A therapeutic system according to claim 44, wherein the medical device is a catheter.

49. A therapeutic system according to claim 44, wherein the medical device is selected from the group consisting of an IV catheter, a central venous line, an arterial catheter, a peripheral catheter, a dialysis catheter, an external fixator pin, peritoneal dialysis catheter, an epidural catheter, a chest tube, and a gastronomy feeding tube.

50. A therapeutic system according to claim 44, wherein the delivery assembly comprises an optical fiber disposed within a catheter, wherein the therapeutic system is configured and arranged to inhibit or prevent a biofilm on a lumen of the catheter when the catheter is placed within patient tissue.

51. A therapeutic system according to claim 28, wherein the delivery assembly includes a hollow waveguide.

52. A therapeutic system according to claim 28, wherein the delivery assembly comprises a free beam optical system.

53. A therapeutic system according to claim 52, wherein the delivery assembly comprises one or more collimating lenses for receiving optical radiation from the an optical radiation generation device and sending the optical radiation to the application region.

54. The therapeutic system of claim 28, wherein the optical radiation is provided for a time (Tn) of from about 50 to about 300 seconds.

55. The therapeutic system of claim 28, wherein the optical radiation is provided for a time (Tn) of from about 75 to about 200 seconds.

56. The therapeutic system of claim 28, wherein the optical radiation is provided for a time (Tn) of from about 100 to about 150 seconds.

57. The therapeutic system of claim 28, wherein the optical radiation is provided for a time (Tn) of from about 100 to about 450 seconds.

58. The therapeutic system of claim 28, wherein the NIMELS dosimetry provides an energy density from about 100 J/cm² to about 500 J/cm².

59. The therapeutic system of claim 28, wherein the NIMELS dosimetry provides an energy density from about 175 J/cm² to about 300 J/cm².

60. The therapeutic system of claim 28, wherein the NIMELS dosimetry provides an energy density from about 200 J/cm² to about 250 J/cm².

61. The therapeutic system of claim 28, wherein the NIMELS dosimetry provides an energy density from about 300 J/cm² to about 700 J/cm².

62. The therapeutic system of claim 28, wherein the NIMELS dosimetry provides an energy density from about 300 J/cm² to about 500 J/cm².

63. The therapeutic system of claim 28, wherein the NIMELS dosimetry provides an energy density from about 300 J/cm² to about 450 J/cm².

64. The therapeutic system of claim 28, wherein the medical device includes an optical catheter controller.
