a computer processor module operatively coupled to the computer memory storage module, wherein the computer processor module is configured to execute the computer programming code to perform the following operations;
detecting an interaction between at least one patient and at least one provider and optionally at least one user;
receiving an input data stream from the interaction;
extracting the received input data stream to generate a raw information;
interpreting the raw information, wherein the interpretation comprises;
converting the raw information to a provider stream of information and a patient stream of information, andclassifying the provider stream of information and patient stream of information using an artificial intelligence module;
andgenerating an output information for the interaction based upon the interpretation of the classified information.

Embodiments of the disclosure provide systems, methods and apparatus for creating understanding, and generating summaries and action item from an interaction between a patient, a provider and optionally a user.
