1. A computer-implemented method to determine one or more parameters associated with a physiological condition of a patient, comprising:

  • receiving, using one or more processors, mean glucose value information of a patient based on a predetermined time period;

    receiving, using the one or more processors, a current HbA1C level of the patient and a target HbA1C level of the patient;

    receiving, using the one or more processors, one or more patient specific parameters;

    determining, using the one or more processors, a correlation between the received mean glucose value information and the received current and target HbA1C levels;

    determining, using the one or more processors, a rate of glycation of the patient based at least in part on the determined correlation between the received mean glucose value information and the received current HbA1C level;

    applying, using the one or more processors, the received one or more patient specific parameters to the determined correlation between the received mean glucose value information and the received current HbA1C level;

    updating, using the one or more processors, the target HbA1C level based on the determined rate of glycation of the patient and the application of the received one or more patient specific parameters to the determined correlation; and

    determining, using the one or more processors, one or more parameters associated with the physiological condition of the patient based on the updated target HbA1C level, wherein determining the one or more parameters associated with the physiological condition of the patient includes one or more of modifying a current alarm setting, modifying a current target threshold setting related to monitored analyte levels, or modifying a medication intake level.
